摘　　要：目的评估不同剂量瑞马唑仑复合瑞芬太尼麻醉对老年直肠癌患者术后意识恢复的影响。方法90例老年直肠癌患者均接受瑞马唑仑复合瑞芬太尼麻醉,根据使用剂量的差异分为对照组46例(高剂量)和观察组44例(低剂量)。比较两组患者术后意识恢复情况、血流动力学指标[平均动脉压(MAP)、心率(HR)]和不良反应发生情况。结果观察组患者自主呼吸时间、意识恢复时间、导管拔除时间及恢复室停留时间均明显短于对照组,差异均有统计学意义(P﹤0.05)。麻醉诱导前(T1)、气管插管后(T2)、手术结束时(T3),两组患者MAP、HR比较,差异均无统计学意义(P﹥0.05)。观察组患者不良反应总发生率低于对照组,差异有统计学意义(P﹤0.05)。结论不同剂量瑞马唑仑复合瑞芬太尼麻醉可维持老年直肠癌患者血流动力学稳定,但低剂量应用可缩短患者术后恢复时间,安全性更高。Objective To evaluate the effect of different doses of remimazolam combined with remifentanil anesthesia on postoperative consciousness recovery in elderly patients with rectal cancer.Method The clinical data of 90 elderly rectal cancer patients who received remimazolam combined with remifentanil anesthesia were collected,and were divided into control group(high dose,n=46)and observation group(low dose,n=44)according to the dosage.The postoperative recovery of consciousness,hemodynamic indexes[mean arterial pressure(MAP),heart rate(HR)]and adverse reactions were compared between the two groups.Result The spontaneous breathing time,consciousness recovery time,catheter removal time and recovery room stay in the observation group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in MAP and HR between the two groups before anesthesia induction(T1),after tracheal intubation(T2)and at the end of operation(T3)(P>0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Different doses of remimazolam combined with remifentanil can maintain the hemodynamic stability in elderly rectal cancer patients,but low dose of remimazolam combined with remifentanil anesthesia can shorten the postoperative recovery time and has higher safety.
