拇指甲下血管球瘤致甲板畸形1例

摘  要:患者女性,40岁,左手拇指指甲形态异常伴持续性疼痛3年。皮肤科查体显示:左拇指甲板表面可见宽约0.4 cm纵嵴,远端甲床粗糙变形,伴甲板溶解,甲周皮肤未见明显异常。此外,具有典型的定位性压痛、阵发性剧痛、冷敏感以及Love试验阳性,高度提示甲下血管球瘤。行手术切除治疗,组织病理学检查示:肿瘤组织由大量管径不一的微血管构成,血管周围环绕多层排列的上皮样细胞,细胞呈梭形或圆形,胞质淡染嗜酸性,核轻度增大,偶见核分裂象。诊断:甲下血管球瘤。术后1个月复查,患者疼痛等不适感消失,术后1年随访见指甲完全恢复正常。A 40-year-old female patient presented with a 3-year history of left thumb nail deformity accompanied by persistent pain.Dermatological examination revealed a 0.4 cm-wide longitudinal ridge on the nail plate surface,with distal nail bed roughness,deformation,and partial dissolution.The periungual skin showed no significant abnormalities.The patient exhibited characteristic clinical features including localized tenderness,paroxysmal severe pain,cold sensitivity,and a positive Love's test,strongly suggestive of subungual glomus tumor.Surgical excision was performed,and histopathological analysis demonstrated tumor tissue composed of numerous microvessels with varying calibers.These vessels were surrounded by multilayered epithelioid cells arranged concentrically,displaying spindle to round morphology with pale eosinophilic cytoplasm,slightly enlarged nuclei,and occasional mitotic figures.The diagnosis of subungual glomus tumor was confirmed.At the 1-month postoperative follow-up,the patient reported complete resolution of pain and related symptoms.A 12-month follow-up revealed full restoration of normal nail morphology and structure.
