摘　　要：目的探讨Toll样受体3(TLR3)在不同结直肠组织中的表达,分析其表达水平与结直肠息肉进展及癌变的关系。方法利用TCGA数据库及CPTAC数据库,分析TLR3在正常结直肠组织和结直肠癌(CRC)中的表达情况及CRC患者的生存情况。收集2022年1月至2023年12月结直肠息肉、结直肠腺癌及对应癌旁组织石蜡标本共计156例(82例结直肠息肉、37例腺癌和37例癌旁组织),采用免疫组化法检测TLR3的表达水平,并分析其与CRC临床病理特征的关系。结果生物信息学分析显示,TLR3在CRC组织中的表达水平低于正常组织,且TLR3低表达患者的预后更差(P<0.05)。免疫组化结果显示,TLR3在CRC组织和癌旁组织中的阳性表达率分别为22/37(59.5%)和32/37(86.5%),差异有统计学意义(P<0.05)。在增生性息肉、腺瘤性息肉和腺癌组织中,TLR3的表达量逐渐降低,差异有统计学意义(P<0.05)。TLR3的表达水平与肿瘤直径和分化程度有关(P<0.05);性别、年龄、肿瘤位置、浸润深度、淋巴结转移、TNM分期、神经及血管侵犯与TLR3表达无关(P>0.05)。结论TLR3在肿瘤组织中的表达量低于正常组织,并且随着“息肉—腺瘤—腺癌”的进展,TLR3的表达量呈递减趋势,低表达患者的预后不良。Objective To investigate the expression of Toll like receptor 3(TLR3)in normal colorectal mucosa,colorectal polyps and adenocarcinoma,and further analyze the relationship between its expression level and the progression and carcinogenesis of colorectal polyps.Methods Bioinformatics was used to analyze the expression and survival status of TLR3 in normal colorectal mucosa and colorectal cancer from TCGA and CPTAC databases.A total of 156 paraffin embedded specimens of colorectal polyps,colorectal adenocarcinoma and corresponding adjacent tissues(82 colorectal polyps,37 adenocarcinoma and 37 normal adjacent colon mucosa)from January 2022 to December 2023 were collected.Immunohistochemistry was used to detect the expression level of TLR3 in the above tissues and the relationship of TLR3 with clinicopathological characteristics of colorectal adenocarcinoma was also analyzed.Results Bioinformatics analysis showed that the expression of TLR3 in colorectal adenocarcinoma tissues was significantly lower than that in normal tissues,and patients with low TLR3 expression had a worse prognosis(P<0.05).Immunohistochemical results indicated that the positive expression rates of TLR3 in colorectal adenocarcinoma tissues and adjacent tissues were 22/37(59.5%)and 32/37(86.5%),respectively,with statistically significant difference(P<0.05).The expression of TLR3 gradually decreased in hyperplastic polyps,adenomatous polyps and adenocarcinoma tissues,with statistically significant difference(P<0.05).The expression level of TLR3 was related to tumor diameter and differentiation degree(P<0.05);gender,age,tumor location,depth of invasion,lymph node metastasis,TNM stage,nerve and vascular invasion were not related to TLR3 expression(P>0.05).Conclusion The expression level of TLR3 in tumor tissues is lower than that in normal tissues,and the expression of TLR3 decreases gradually with the progression of“polyp-adenoma-adenocarcinoma”.Patients with low expression of TLR3 have a poor prognosis.
