175例多原发癌的临床特征与预后分析

摘　　要：目的回顾性分析多原发癌(MPMTs)患者的临床病理特征和预后情况。方法回顾性分析2009年至2019年MPMTs患者的临床资料。对入组患者的一般资料、治疗情况、基因谱和预后进行分析。结果从15268例肿瘤患者中筛选出175例(1.2%)MPMTs。其中,男性76例,女性99例。第一、第二和第三原发癌的诊断中位年龄分别为54.9岁、63.5岁和59.4岁;汉族164例,少数民族11例。同时性多原发癌(SMPMTs)48例(27.4%),异时性多原发癌(MMPMTs)127例(72.6%),第三原发癌9例(5.1%)。肿瘤分布包括消化道,泌尿系统,生殖系统,肺、乳腺和头颈部。消化道肿瘤是最常见的原发肿瘤,其次为乳腺肿瘤。13例MPMTs患者接受基因检测,共检测出11个特异性基因突变。全组患者的治疗方式包括手术、放疗、化疗、内分泌治疗、靶向治疗、免疫治疗、介入治疗、最佳支持治疗和中医药治疗。全组中位总生存时间(OS)为72.0个月。11例少数民族患者的中位OS为72.0个月。MMPMTs与SMPMTs患者的中位OS分别为72.0个月和72.0个月(P=0.23)。结论MPMTs患者大多为中老年人,原发肿瘤多为消化道肿瘤和乳腺肿瘤,新疆地区不同民族间MPMTs患者OS的差异无显著性。Objective To retrospectively analyze the clinical and pathological characteristics and prognosis of patients with multiple primary cancers(MPMTs).Methods A retrospective analysis was conducted on the clinical data of patients with MPMTs from 2009 to 2019.The general information,treatment status,genetic profile and prognosis were analyzed.Results A toal of 175 cases(1.2%)of MPMTs were screened from 15268 cancer patients.Among them,there were 76 males and 99 females.The median ages when the first,second,and third primary cancer diagnosed were 54.9 years,63.5 years,and 59.4 years,respectively.There were 164 cases of Han ethnicity and 11 cases of ethnic minorities.Among the 175 cases,there were 48 cases(27.4%)of synchronous multiple primary cancers(SMPMTs)and 127 cases(72.6%)of metachronous multiple primary cancers(MMPMTs).The third primary cancer occurred in nine cases(5.1%).The distribution of tumors included digestive tract,urinary system,reproductive system,lung,breast,and head and neck.Digestive tract tumors were the most common primary tumors,followed by breast tumors.Thirteen patients with MPMTs underwent genetic testing,and a total of 11 specific gene mutations were detected.The treatment methods included surgery,radiotherapy,chemotherapy,endocrine therapy,targeted therapy,immunotherapy,interventional therapy,best supportive therapy and traditional Chinese medicine.The median overall survival(OS)for the entire group was 72.0 months.The median OS of 11 ethnic minority patients was 72.0 months.The median OS of MMPMTs and SMPMTs patients were 72.0 months and 72.0 months,respectively(P=0.23).Conclusion Most MPMTs patients are middle-aged and elderly,and the primary tumors are mostly gastrointestinal and breast tumors.There is no significant difference in the OS among MPMTs patients of different ethnic groups in Xinjiang.
