MRI联合血清B7-H4、TFF3检测对卵巢肿瘤良恶性的诊断价值

摘  要:目的探讨磁共振成像(MRI)联合血清共刺激因子B7-同源体4(B7-H4)、三叶因子3(TFF3)检测对卵巢肿瘤良恶性的诊断价值。方法收集2021年2月至2024年1月汉中市人民医院收治的164例卵巢肿瘤患者,根据术后病理检测结果分为恶性组(57例)和良性组(107例)。通过受试者工作特征(ROC)曲线分析血清B7-H4、TFF3对卵巢肿瘤良恶性的诊断价值;采用卡方检验分析MRI联合血清B7-H4、TFF3诊断卵巢肿瘤良恶性的价值;采用多因素Logistic回归模型分析影响卵巢肿瘤良恶性的因素。结果恶性组血清B7-H4、TFF3水平显著高于良性组(P<0.05)。MRI诊断卵巢肿瘤良恶性的曲线下面积(AUC)为0.817,灵敏度为82.46%,特异度为75.70%,准确度为78.05%。血清B7-H4诊断卵巢肿瘤良恶性的AUC为0.834,截断值为66.641μg/L,灵敏度和特异度分别为80.70%和77.57%;血清TFF3诊断卵巢肿瘤良恶性的AUC为0.855,截断值为279.974μg/L,灵敏度和特异度分别为80.70%和76.64%。MRI联合血清B7-H4、TFF3诊断恶性卵巢肿瘤的AUC为0.865,灵敏度和特异度分别为94.74%和74.77%,准确度为81.71%。多因素Logistic回归分析显示,血清B7-H4、TFF3及肿瘤成分是影响卵巢肿瘤良恶性的因素(P<0.05)。结论MRI联合血清B7-H4、TFF3检测可提高对卵巢肿瘤良恶性的诊断价值。Objective To analyze the diagnostic value of magnetic resonance imaging(MRI)combined with serum co-stimulatory factor B7 homolog 4(B7-H4)and trefoil factor 3(TFF3)detection for benign and malignant ovarian tumors.Methods From February 2021 to January 2024,A total of 164 patients with ovarian tumors who accepted by Hanzhong People's Hospital were collected.Based on postoperative pathological examination results,the patients were separated into a malignant group(57 cases)and a benign group(107 cases).Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of serum B7-H4 and TFF3 for benign and malignant ovarian tumors.The value of MRI combined with serum B7-H4 and TFF3 in diagnosing benign and malignant ovarian tumors was calculated by Chi-square test.Multivariate Logistic regression model was applied to analyze the factors affecting the malignancy of ovarian tumors.Results The serum B7-H4 and TFF3 levels in the malignant group were obviously higher than those in the benign group(P<0.05).The area under the curve(AUC)of MRI in diagnosing benign and malignant ovarian tumors was 0.817,and the sensitivity,specificity,and accuracy were 82.46%,75.70%and 78.05%,respectively.The AUC of serum B7-H4 for diagnosing benign and malignant ovarian tumors was 0.834,with a cutoff value of 66.641μg/L,sensitivity and specificity of 80.70%and 77.57%,respectively.The AUC of serum TFF3 for diagnosing benign and malignant ovarian tumors was 0.855,with a cutoff value of 279.974μg/L,sensitivity and specificity of 80.70%and 76.64%,respectively.The AUC of MRI combined with serum B7-H4 and TFF3 in diagnosing malignant ovarian tumors was 0.865,and the sensitivity and specificity were 94.74%and 74.77%,respectively,with an accuracy of 81.71%.Multivariate Logistic regression analysis showed that B7-H4,TFF3 and tumor component were factors that affected the malignancy of ovarian tumors(P<0.05).Conclusion MRI combined with serum B7-H4 and TFF3 detection can improve the diagnostic value of benign and malignant ovari
