摘  要:目的 探讨声触诊弹性成像技术(sound touch elastography,STE)联合能量多普勒(power doppler ultrasound,PDU)在前列腺外周带病变中的诊断价值及与Gleason评分的关系。方法 选取前列腺外周带病变87例,根据穿刺病理结果分为前列腺癌(PCa)组50例和前列腺增生(BPH)组37例,比较两组间Emax、Emean及PDU差异,分析各指标与Gleason评分的关系,绘制ROC曲线比较不同指标对外周带PCa的诊断效能。结果 PCa组的Emax、Emean、PDU均大于BPH组,差异均具有统计学意义(P <0.001)。各指标与Gleason评分均呈正相关(P <0.05),其中Emax与Gleason评分相关性最强。中高危组的Emax、Emean大于低危组,差异有统计学意义(P <0.001),两组的PDU差异无统计学意义(P>0.05);低危组的PDU大于BPH组,差异有统计学意义(P <0.001),两组的Emax、Emean差异无统计学意义(P>0.05);单独应用Emax、Emean及PDU检测PCa的曲线下面积(AUC)分别为0.815、0.801、0.794,三指标联合应用的AUC为0.921,高于各指标单独应用,差异有统计学意义(P <0.05)。结论 STE、PDU对外周带PCa具有一定诊断价值,并能预测癌症的病理分级,且两者联合应用效能更高。Objective To analyze the diagnostic value of sound touch elastography(STE)in combination with power doppler ultrasound(PDU)in lesions of the prostate peripheral zone,and to explore its correlation with Gleason score.Methods A to-tal of 87 cases of lesions in the prostate peripheral zone were selected and categorized into a prostate cancer(PCa)group with 50 cases and a benign prostatic hyperplasia(BPH)group with 37 cases based on the results of biopsy pathology.Comparisons were conducted between the two groups in terms of Emax,Emean,and PDU.The relationship between each parameter and the Glea-son score was analyzed.ROC curves were also plotted to compare the diagnostic performance of these parameters for PCa in the peripheral zone.Results The PCa group demonstrated significantly higher values of Emax,Emean,and PDU compared to the BPH group,with statistical significance(P<0.001).Each parameter showed a positive correlation with the Gleason score(P<0.05),with Emax exhibiting the strongest correlation.The middle-and high-risk PCa groups had significantly higher Emax and Emean values than the low-risk group(P<0.001),while no statistically significant difference was observed in PDU between these two risk groups(P>0.05).The PDU of the low-risk PCa group was significantly higher than that of the BPH group(P<0.001),but there was no statistically significant difference in Emax and Emean between them(P>0.05).When Emax,Emean,and PDU were used individually for PCa detection,the areas under the curve(AUCs)were 0.815,0.801,0.794,The AUC for the combined application of these three parameters was 0.921,which was significantly higher than that of each parameter used alone(P<0.05).Conclusion STE and PDU possess diagnostic significance for PCa in the peripheral zone of the prostate,with the capability to predict the pathological grading of the cancer.Furthermore,the combined application of the two techniques demonstrates enhanced efficacy.
