脑卒中患者居家康复支持性照护需求的质性研究

摘　　要：目的采用质性研究方法深入剖析脑卒中患者居家康复支持性照护需求。方法采用目的抽样法,选取2023年9月—2024年1月就诊于南京大学医学院附属鼓楼医院神经内科门诊或康复科的脑卒中患者为调查对象。样本量以信息“饱和”为标准,最终共纳入15例受访者。根据访谈提纲,对受访者进行面对面半结构式访谈。运用Colaizzi 7步分析法进行主题分析和提炼。结果最终共提炼出5个主题,分别为多维信息整合需求(导航式康复信息支持需求、自我健康管理过程中的信息补偿需求、便捷化医患互动模式需求)、身体功能恢复需求(实现功能独立的需求、缓解躯体不适症状的需求)、情感支持与关怀需求(外部正向情感反馈需求、监督与鞭策的需求、被理解与尊重的需求)、回归社会的需求(“返轨”社交网络需求、维持社会角色功能的需求)、现实康复环境支持需求(优质基层康复资源的支持需求、无障碍居住环境适宜性改造的需求、医保支持及财政援助需求)。结论脑卒中患者在居家康复过程中存在多维信息整合需求、身体功能恢复需求、情感支持与关怀需求、回归社会的需求、现实康复环境支持需求。Objective To deeply analyze the supportive care needs of home rehabilitation for stroke patients using qualitative research methods.Methods Using purposive sampling method,stroke patients who visited the Neurology Outpatient Department or Rehabilitation Department of Nanjing Drum Tower Hospital,the Affiliated Hospital of Nanjing University Medical School from September 2023 to January 2024 were selected as the survey subjects.The sample size was based on information"saturation",and a total of 15 respondents were included.According to the interview outline,a face-to-face semi-structured interview was conducted with the interviewees.Colaizzi 7-step analysis method was used to analyze and extract the topic.Results A total of 5 themes were finally extracted,which were multidimensional information integration needs(navigational rehabilitation information support needs,information compensation needs in the process of self-health management,and convenient doctor-patient interaction needs),physical function recovery needs(the need to achieve functional independence,the need to relieve symptoms of physical discomfort),emotional support and care needs(the need for external positive emotional feedback,the need for supervision and encouragement,the need to be understood and respected),the need to return to society("back on track"social network needs,the need to maintain social role functions),and the need for realistic rehabilitation environment support(demand for support of high-quality primary rehabilitation resources,demand for suitable renovation of barrier-free living environment,demand for medical insurance support and financial assistance).Conclusion In the process of home rehabilitation of stroke patients,there are multidimensional information integration needs,physical function recovery needs,emotional support and care needs,social return needs,and realistic rehabilitation environment support needs.
