摘 要:目的模拟上颌埋伏中切牙及其邻牙在埋伏牙的不同阻生类型下加载不同的力值时的运动,分析牙齿的瞬时位移和牙周膜应力分布特点。方法应用有限元分析法,模拟五种不同的上颌中切牙埋伏阻生类型:高位、水平位、倒置位、近中斜位、远中斜位,并在模型上施加0.3 N、0.6 N、0.9 N和1.2 N的力,以观察牙齿的瞬时位移及牙周应力分布。结果随着加载力值的逐渐增加,三颗牙齿的瞬时位移量和牙周Von Mises应力值也逐渐增加。各工况下,埋伏中切牙的瞬时位移方向基本沿着牵引力的方向,而相邻侧切牙和对侧中切牙均有向牵引力方向倾斜移动的趋势。各工况下,埋伏中切牙的牙周应力集中区域均出现在根尖处和牙颈部,而对侧中切牙和相邻侧切牙的牙周应力分布较为均匀。结论埋伏牙的阻生类型不同,正畸牵引时的牙周应力分布也会不同,临床上应根据不同阻生类型下埋伏牙牙周组织所能承受的力值,合理选择牵引力的大小。Objective To simulate the movement of maxillary buried mesial incisors and their neighboring teeth when loaded with different force values under different types of obstruction of the buried teeth,and analyse the instantaneous displacement of the teeth and the characteristics of the periodontal stress distribution.Methods Finite element analysis method was applied to simulate five different types of maxillary mesial incisor ambulatory obstruction:high,horizon-tal,inverted,proximal mesial oblique,and distal mesial oblique,and forces of 0.3 N,0.6 N,0.9 N,and 1.2 N were applied to the models,the distribution of periodontal stress and the in-stantaneous displacement of the teeth was observed.Results With the gradual increase of the loading force values,the instantaneous displacements of the three teeth and the periodontal Von Mises stress values also gradually increased.The direction of the instantaneous displacement of the buried mesial incisors was basically along the direction of the traction force in all working conditions,whereas the adjacent lateral incisors and the contralateral mesial incisors had a tendency to move inclined toward the direction of the traction force.In all working conditions,the periodontal stress concentration areas of the buried mesial incisors appeared at the apices and necks,while the periodontal stress distribution of the contralateral mesial incisors and adja-cent lateral incisors was more uniform.Conclusion The periodontal stress distribution during orthodontic traction will be different for different types of obstruction of the ambulatory teeth,and the size of the traction force should be reasonably selected clinically according to the force values that the periodontal tissues of the ambulatory teeth can withstand under different types of obstruction.
