半导体激光联合复方氯己定地塞米松膜对慢性牙周炎患者牙周指标及牙龈沟液炎症指标的影响

摘　　要：目的探讨半导体激光联合复方氯己定地塞米松膜对慢性牙周炎患者牙周指标及牙龈沟液炎症指标的影响。方法选取2023年1月至2024年3月该院接诊的92例慢性牙周炎患者作为受试者,根据随机数字表法将患者分为观察组与对照组,各46例。2组均进行常规清洁及龈下刮治,对照组在此基础上给予复方氯己定地塞米松膜治疗,观察组在对照组的基础上加以半导体激光治疗。比较2组临床疗效、牙周指标、牙龈沟液炎症指标、咀嚼功能评分、牙龈微循环指标及不良反应发生情况。结果观察组总有效率为93.48%,高于对照组的78.26%(P<0.05)。治疗后,2组牙菌斑指数、牙龈指数及牙周袋深度均低于治疗前(P<0.05),且观察组低于对照组(P<0.05)。治疗后,2组龈沟液白细胞介素(IL)-6、IL-8及肿瘤坏死因子(TNF)-α水平均低于治疗前(P<0.05),且观察组均低于对照组(P<0.05)。治疗后,2组咀嚼功能评分、牙龈血流量高于治疗前(P<0.05),且观察组高于对照组(P<0.05)。治疗后,2组血流速度快于治疗前(P<0.05),且观察组快于对照组(P<0.05)。2组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论半导体激光联合复方氯己定地塞米松膜较单独使用复方氯己定地塞米松膜可有效提高慢性牙周炎患者疗效,改善牙周指标、牙龈沟液炎症指标、咀嚼功能及牙龈微循环指标且不增加不良反应发生风险。Objective To investigate the effect of semiconductor laser combined with compound chlorhexidine and dexamethasone membrane on periodontal indexes and gingival sulcus inflammation indexes in patients with periodontitis.Methods Ninety-two patients with chronic periodontitis admitted to the hospital from January 2023 to March 2024 were selected as the subjects.The patients were divided into the observation group and the control group according to random number table method,with 46 cases in each group.Both groups were treated with routine cleaning and subgingival scaling.The control group was treated with compound chlorhexidine and dexamethasone membrane on the basis of the treatment,and the observation group was treated with semiconductor laser on the basis of the control group.The clinical efficacy,periodontal indexes,gingival crevicular fluid inflammation indexes,masticatory function score,gingival microcirculation indexes and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.48%,which was higher than 78.26%of the control group(P<0.05).After treatment,the plaque index,gingival index and periodontal pocket depth of the two groups were lower than those before treatment(P<0.05),and those of the observation group were lower than those of the control group(P<0.05).After treatment,the levels of interleukin(IL)-6,IL-8 and tumor necrosis factor(TNF)-αin gingival crevicular fluid of the two groups were lower than those before treatment(P<0.05),and those in the observation group were lower than those in the control group(P<0.05).After treatment,the masticatory function scores and gingival blood flow of the two groups were higher than those before treatment(P<0.05),and those of the observation group were higher than those of the control group(P<0.05).After treatment,the blood flow velocity of the two groups was faster than that before treatment(P<0.05),and the observation group was faster than the control group(P<0.05).There was no significant difference
