NLR与影像学高负荷评分CSVD患者认知功能障碍的相关性及诊断价值研究

摘　　要：目的探讨中性粒细胞/淋巴细胞比值(NLR)与影像学高负荷评分脑小血管病(CSVD)患者认知功能障碍的关系及其诊断认知障碍的临床价值。方法回顾性分析2018年12月—2022年12月在扬州洪泉医院神经内科接受治疗的378例CSVD患者,根据蒙特利尔认知评估量表(MoCA)评分将患者分为认知障碍组(n=290)和非认知障碍组(n=88)。入院后患者收集患者临床资料并检测相关实验室指标。Pearson相关性分析NLR与MoCA评分的相关性,ROC曲线分析NLR预测CSVD患者认知障碍的价值。结果相关性分析显示NLR与MoCA评分呈显著负相关(r=﹣0.764,P<0.001)。ROC曲线分析NLR预测CSVD患者认知障碍的曲线下面积0.890、灵敏度为81.07%、特异度为82.23%。结论NLR与影像学高负荷评分CSVD患者认知障碍严重程度呈正相关,用于预测此类患者认知障碍具有一定的临床价值。Objective To investigate the relationship between neutrophil/lymphocyte ratio(NLR)and cognitive dysfunction in patients with high imaging load score cerebral small vascular disease(CSVD)its clinical value in diagnosis of cognitive impairment.Methods 378 patients with CSVD who were treated in the Department of Neurology of Yangzhou Hongquan Hospital from December 2018 to December 2022 were retrospectively analyzed and divided into cognitive impairment group(n=290)and non-cognitive impairment group(n=88)according to the Montreal Cognitive Assessment Scale(MoCA)score.After admission,clinical data of patients were collected and relevant laboratory indicators were detected.Pearson correlation analysis was conducted to analyze the correlation between NLR and MoCA score,and ROC curve analysis was conducted to analyze the value of NLR in predicting cognitive impairment in CSVD patients.Results The correlation analysis revealed a significant inverse relationship between the NLR and the Montreal MoCA score(r=-0.764,P<0.001).ROC curve analysis NLR predicted cognitive impairment in CSVD patients with area under the curve of 0.890,sensitivity of 81.07%,specificity of 82.23%.Conclusion NLR demonstrates a positive correlation with the severity of cognitive impairment in CSVD patients exhibiting high imaging load scores,thereby providing significant clinical value for predicting cognitive decline in this patient population.
