摘 要:目的 探讨功能性电刺激联合运动训练在老年脑卒中治疗中的应用效果。方法 选择北京市中关村医院2021年1月至2023年12月128例脑卒中患者,采用随机数字表法分为对照组和研究组,所有患者均接受常规康复训练,对照组64例在此基础上采用等速肌力训练,研究组在对照组基础上实施功能性电刺激,两组治疗时间均为4周。观察两组治疗前、治疗4周后的神经功能、步行能力、肢体功能、神经元特异性烯醇化酶(NSE)水平、认知功能及日常生活能力变化。结果 治疗后研究组美国国立卫生研究院卒中量表(NIHSS)评分低于对照组,最大步行速度(MWS)快于对照组(P<0.05);治疗后研究组Fugl-Meyer评估表(FMA)评分高于对照组(P<0.05);治疗后研究组的血清NSE水平均低于对照组,蒙特利尔认知评估量表(MoCA)评分均高于对照组(P<0.05);治疗后研究组日常生活能力评估量表(ADL)评分高于对照组(P<0.05)。结论 功能性电刺激联合运动训练治疗能够促进老年脑卒中后神经功能、步行能力及肢体功能恢复,改善患者NSE水平及认知功能,提高其日常生活能力。Objective To explore the application effect of functional electrical stimulation combined with exercise training in the treatment of senile stroke.Methods A total of 128 stroke patients with Zhongguancun Hospital from January 2021 to December 2023 were selected and divided into study group and the control group by random number table method.All patients received routine rehabilitation training,64 patients in control group were trained by isokinetic muscle strength training on this basis,and functional electrical stimulation was implemented in the study group on the basis of control group.Both groups were treated for 4 weeks.The changes of nerve function,walking ability,limb function,neuron-specific enolase(NSE)level,cognitive function and daily living ability were observed before and 4 weeks after treatment in both groups.Results The score of National Institutes of Health stroke scale(NIHSS)of the study group were lower than those of the control group after treatment,and the maximum walking speed(MWS)was faster than that of the control group(P<0.05).The score of Fugl-Meyer assessment scale(FMA)of the study group were higher than those of the control group after treatment(P<0.05).The NSE level of the study group were lower than those of the control group,and the score of Montreal cognitive assessment scale(MoCA)were all higher than those of the control group(P<0.05).The score of assessment of daily living(ADL)of the study group were higher than those of the control group after treatment(P<0.05).Conclusions Functional electrical stimulation combined with exercise training can promote the recovery of nerve function,walking ability and limb function,improve the level of NSE and cognitive function,and enhance the ability of daily living in elderly patients after stroke.
