子宫内膜癌患者血清HE4、IGF-1、CA153水平与预后关系

摘　　要：目的:分析子宫内膜癌(EC)患者血清人附睾蛋白4(HE4)、胰岛素样生长因子(IGF)-1、糖类抗原153(CA153)水平及其与预后关系。方法:选取2021年7月-2023年2月在本院择期手术治疗的EC患者112例为EC组,年龄匹配体检健康女性56例为对照组,分别在EC组术前和对照组体检时取外周静脉血检测血清HE4、IGF-1、CA153水平,分析各指标水平与EC患者临床病理及术后1年预后的关系。采用受试者工作特征曲线(ROC)分析术前血清HE4、IGF-1、CA153对预后的预测价值。结果:EC组血清HE4(108.90±12.23 pmol/L)、IGF-1(349.12±48.47μg/L)、CA153(37.24±6.86 U/ml)水平均高于对照组(31.60±3.95 pmol/L、116.25±14.53μg/L、22.54±3.44 U/ml),不同FIGO分期、分化程度、肌层浸润深度、淋巴结转移临床特征的EC患者血清HE4、IGF-1、CA153水平有差异,术后1年预后不良患者血清HE4、IGF-1、CA153水平高于预后良好患者(均P<0.05)。ROC曲线显示,EC患者血清HE4、IGF-1、CA153预测手术预后的曲线下面积(AUC)分别为0.723、0.704、0.726,敏感度分别为58.6%、55.2%、62.1%,特异度为88.0%、75.9%、77.1%,3项指标联合预测的AUC为0.862,敏感度为96.6%,特异度为75.9%,联合检测预测预后的效能高于各指标单独检测(P<0.05)。结论:EC患者血清HE4、IGF-1、CA153水平较高其术后1年内预后较差,血清HE4、IGF-1、CA153指标联合检测可提高预测患者术后预后效能。Objective:To analyze the correlation between the levels of the serum human epididymis protein 4(HE4),insulin-like growth factor(IGF)-1,carbohydrate antigen 153(CA153)and prognosis of women with endometrial cancer(EC)and their prognosis.Methods:A total of 112 women with EC who wanted elective surgery in the hospital were enrolled in study group between July 2021 and February 2023,while 56 healthy women with the matched age who underwent physical examination were enrolled in control group during the same period.The levels of the serum HE4,IGF-1 and CA153 of the women in the study group before surgery and those of the women in the control group during physical examination were detected.The correlation between the levels of the serum HE4,IGF-1 and CA153 of the women with EC and their clinicopathological characteristics and prognosis within 1-year after surgery was analyzed.Receiver operating characteristic(ROC)curve was used to analyze the predictive values of the preoperative serum HE4,IGF-1 and CA153 levels of the women with EC for their prognosis.Results:The levels of the serum HE4(108.90±12.23 pmol/L),IGF-1(349.12±48.47μg/L)and CA153(37.24±6.86 U/ml)of the women in the study group were significantly higher than those(31.60±3.95 pmol/L,116.25±14.53μg/L and 22.54±3.44 U/ml)of the women in the control group.In the study group,there were significant difference of the levels of the serum HE4,IGF-1 and CA153 among the patients with different endometrial cancer FIGO stages,among the patients with different degree of endometrial cancer differentiation,among the patients with different depth of endometrial cancer myometrial invasion and among the patients with different clinical characteristics of lymph node metastasis.In the study group,the serum levels of HE4,IGF-1and CA153of the patients with poor prognosis within 1year after surgery were significantly higher than those of the patients with good prognosis(all P<0.05).ROC curve showed that the area under the curve(AUC)of the serum HE4level,IGF-1level and CA153le
