前哨淋巴结示踪联合病理超分期在Ⅰ期子宫内膜癌盆腔淋巴结转移评估中的可行性

摘　　要：目的:探究在子宫内膜癌(早期)中应用病理超分期结合前哨淋巴结(SLN)示踪对盆腔淋巴结转移评估的可行性。方法:选择2022年1月-2023年12月本院治疗的子宫内膜癌患者共35例临床资料,均行进腹后宫颈注射2ml体积的亚甲蓝,先予以手术切除再实施相应的病理超分期检测。结果:35例子宫内膜癌患者中29例SLN被成功检测出,总检出率82.8%,单侧及双侧检出率分别是28.6%和54.3%。20例高危型子宫内膜癌中SLN检出率85.0%,15例低危型子宫内膜癌中SLN检出率是80.0%。SLN检测盆腔淋巴结微转移的灵敏度、阳/阴性的预测值均为100%,而假阴性率则为0。结论:子宫内膜癌(早期)通过病理超分期结合SLN示踪是可行的,同时能够发现传统病理染色无法发现的淋巴结微转移情况,从而为精准治疗提供指导。Objective:To explore the feasibility of the pathologic super-staging combined with the sentinel lymph node(SLN)tracing used in the patients with early endometrial carcinoma.Methods:The clinical data of 35 patients with endometrial cancer who were treated in the hospital from January 2022 to December 2023 were selected in this study.After opening the abdomen,2ml of methylene blue injected into the cervix of these patients,followed by the surgical resection of the endometrial carcinoma,and the subsequent pathological super-staging were detected.Results:Among 35 patients with endometrial cancer,there were 29 cases with SLN detected successfully,with an overall detection rate of 82.8%,and the detection rates of the unilateral and bilateral lymph nodes were 28.6%and 54.3%.In 20 patients with high-risk endometrial cancer,the SLN detection rate was 85.0%,and in 15 patients with low-risk endometrial cancer,the SLN detection rate was 80%.The sensitivity and the positive/negative predictive values of SLN detection(micrometastasis of pelvic lymph nodes)were all 100%,while the false-negative rate was 0%.Conclusion:The application of the pathological super-staging combined with SLN tracing is feasible in the patients with early endometrial carcinoma,and which can detect the lymph node micrometastases that cannot be detected by traditional pathological staining,so as to provide more evidences for the accurate treatment of the patients.
