NLR、PLR和血清CA125水平鉴别诊断卵巢良恶性肿瘤价值

摘　　要：目的:探究中性粒细胞和淋巴细胞比率(NLR)、血小板和淋巴细胞比值(PLR)和血清糖类抗原(CA125)水平鉴别诊断卵巢良恶性肿瘤价值。方法:选择2020年6月-2024年6月本院确诊的卵巢良性肿瘤患者45例和卵巢恶性肿瘤患者45例,分为良性组和恶性组,比较两组NLR、PLR、CA125水平,使用受试者工作特征(ROC)曲线分析NLR、PLR、CA125水平诊断卵巢恶性肿瘤价值,使用Spearman法分析NLR、PLR和血清CA125水平与卵巢恶性肿瘤的相关性;与卵巢恶性肿瘤病理特征的相关性。结果:恶性组CA125(39.25±14.94 U/ml)、NLR(2.32±0.65)、PLR(175.62±44.81)均高于良性组(18.50±4.52 U/ml、1.46±0.43、126.67±12.54)(均P<0.05);NLR、PLR、CA125诊断卵巢恶性肿瘤的曲线下面积(AUC)分别为0.867、0.877、0.859,3项联合诊断的AUC(0.975)显著提高(P<0.05);NLR、PLR、CA125水平与卵巢恶性肿瘤发生呈正相关,且与肿瘤大小有关(均P<0.05)。结论:NLR、PLR、CA125水平对卵巢恶性肿瘤有一定鉴别诊断价值,且联合诊断价值提高。Objective:To investigate the values of the neutrophil to lymphocyte ratio(NLR),the platelet to lymphocyte ratio(PLR)and the serum carbohydrate antigen 125(CA125)level of patients for differential diagnosing their malignant or benign ovarian tumors.Methods:45 patients with benign ovarian tumors(in group A)and 45 patients with malignant ovarian tumors(in group B)who had been definite diagnoses from June 2020 to June 2024 in the hospital were selected in this study.The values of the NLR and PLR,and the CA125 level of the patients were compared between the two groups.Receiver operator characteristic(ROC)curve was used to analyze the values of the NLR and PLR,and the CA125 level of the patients for diagnosing their malignant ovarian tumors.Spearman method was applied to analyze the correlation between of the NLR and PLR values,and the CA125 level of the patients and their malignant ovarian tumors and the pathological characteristics of their malignant ovarian tumors.Results:The CA125 level(39.25±14.94 U/ml),the NLR value(2.32±0.65)and the PLR value(175.62±44.81)of the patients in group B were significantly higher than those(18.50±4.52 U/ml,1.46±0.43 and 126.67±12.54)of the patients in group A(all P<0.05).The area under the curve(AUC)of the NLR and PLR values,and the CA125 level of the patients for diagnosing their malignant ovarian tumors were 0.867,0.877 and 0.859,respectively,and the AUC of the NLR and PLR values combined with the CA125 level of the patients for diagnosing their malignant ovarian tumors was 0.975,and which had increased significantly(P<0.05).The NLR and PLR values,and the CA125 level of the patients were positively correlated with the occurrence of their malignant ovarian tumors,and which were also related to the malignant ovarian tumor size of the patients(all P<0.05).Conclusion:The NLR and PLR values,and the CA125level of the patients for differential diagnosing their malignant ovarian tumors have certain values,and the value of the NLR and PLR values combined with the CA125level of the pati
