摘　　要：目的探讨基于Bio-OssGeistlich的拔牙位点保存术联合不同软组织处理方式对口腔后牙种植修复效果、牙槽骨骨量、种植体成功率的影响。方法前瞻性选取2019年1月至2023年3月于郑州大学第一附属医院就诊且需行口腔后牙种植的70例患者纳入研究,按随机数表法分为A组(患牙46颗)和B组(患牙45颗)各35例。两组患者均接受Bio-OssGeistlich拔牙位点保存术,A组患者在此基础上联合Bio-Guide生物膜,B组患者则联合可吸收性明胶海绵。比较两组患者修复6个月后的种植体成功率、种植修复效果、美观度,拔牙前、修复6个月后的咀嚼功能、牙槽骨骨量、牙周健康状态和修复1年后的并发症发生率。结果修复6个月后,A组患者的种植成功率为100.00%,明显高于B组的84.44%,差异有统计学意义(P<0.05);修复6个月后,A组患者的外观、边缘贴合性、颜色改变修复效果明显优于B组,差异均有统计学意义(P<0.05);修复6个月后,A组患者的咬合力、咀嚼效率及牙槽骨宽度、高度、密度明显高于B组,差异均有统计学意义(P<0.05);修复6个月后,A组患者的牙龈指数、牙周探诊深度、菌斑指数、出血指数明显低于B组,软组织形态、颜色、质地评分明显高于B组,差异均有统计学意义(P<0.05);修复1年后A组患者的并发症总发生率为9.38%,略低于B组的22.58%,但差异无统计学意义(P>0.05)。结论Bio-Oss Geistlich拔牙位点保存术中使用Bio-Guide生物膜覆盖可有效提高种植体成功率及美观度,延缓牙槽骨吸收,改善牙周状态及牙齿功能。Objective To investigate the effects of Bio-Oss Geistlich-based tooth extraction site preservation combined with different soft tissue management techniques on oral posterior dental implant restoration outcomes,alveo-lar bone volume,and implant success rate.Methods A prospective study was conducted on 70 patients requiring poste-rior dental implant restoration at The First Affiliated Hospital of Zhengzhou University from January 2019 to March 2023.Patients were randomly divided into group A(46 teeth)and group B(45 teeth),each with 35 patients.Both groups of patients underwent Bio-Oss Geistlich tooth extraction site preservation.Patients in group A additionally received Bio-Guide collagen membrane coverage,while those in group B received absorbable gelatin sponge coverage.Implant success rate,restoration outcomes,aesthetics,chewing function,alveolar bone volume,periodontal health,and 1-year complication rates were compared between the two groups.Results At 6 months after restoration,the implant success rate in group A was 100.00%,significantly higher than group B’s 84.44%(P<0.05).Group A exhibited superior restora-tion outcomes in appearance,margin adaptation,and color stability compared to group B(P<0.05).At 6 months after res-toration,patients in group A demonstrated significantly higher occlusal force,chewing efficiency,alveolar bone width,height,and density than those in group B(P<0.05).Periodontal indices(gingival index,probing depth,plaque index,bleeding index)were significantly lower in group A(P<0.05),while soft tissue morphology,color,and texture scores were significantly higher(P<0.05).The 1-year complication rate in group A(9.38%)was slightly lower than that in group B(22.58%),but the difference was not statistically significant(P>0.05).Conclusion Combining Bio-Oss Geistlich site preservation with Bio-Guide collagen membrane enhances implant success rate and aesthetic outcomes,mitigates al-veolar bone resorption,and improves periodontal health and masticatory function.
