摘 要:目的探讨血清Krüpple样因子5(KLF5)mRNA、跨膜蛋白64(TMEM64)mRNA与肝癌患者血清甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原199(CA199)的相关性,并进一步分析其诊断价值。方法按照2:1:1前瞻性选取2022年1月至2024年8月河南省肿瘤医院收治的170例肝癌患者(肝癌组)、85例良性肝脏疾病患者(良性组)和85例健康体检人群(对照组)。比较三组受检者的血清AFP、CEA、CA199、KLF5 mRNA、TMEM64 mRNA,采用Pearson相关性分析KLF5 mRNA、TMEM64 mRNA与AFP、CEA、CA199相关性,采用单因素和多因素Logistic回归分析肝癌的影响因素,采用受试者工作特征(ROC)曲线及其曲线下面积(AUC)评估AFP、CEA、CA199、KLF5 mRNA、TMEM64 mRNA以及AFP+CEA+CA199与KLF5 mRNA+TMEM64 mRNA诊断肝癌的价值,并对比不同方案预测价值。结果肝癌组患者AFP、CEA、CA199、KLF5 mRNA、TMEM64 mRNA分别为(35.82±11.93)ng/mL、(6.24±2.06)ng/mL、(58.40±19.33)ng/mL、8.86±2.94、6.95±2.30,良性组分别为(12.30±4.09)ng/mL、(3.59±1.19)ng/mL、(3.59±1.19)ng/mL、4.88±1.60、3.11±1.02,对照组分别为(11.66±3.81)ng/mL、(3.25±1.08)ng/mL、(17.99±5.51)ng/mL、4.71±1.55、2.89±0.95;肝癌组患者的上述各项指标明显高于良性组和对照组,差异均有统计学意义(P<0.05),而良性组和对照组比较差异均无统计学意义(P>0.05)。Pearson相关性分析结果显示,肝癌组患者KLF5 mRNA、TMEM64 mRNA与AFP、CEA、CA199呈正相关(r=0.795、0.785、0.789;0.811、0.791、0.771,P<0.05);多因素Logistic回归分析结果显示,KLF5 mRNA升高、TMEM64 mRNA升高均是肝癌的独立相关危险因素(P<0.05)。ROC曲线分析结果显示,AFP+CEA+CA199的AUC大于AFP、CEA、CA199,KLF5 mRNA+TMEM64 mRNA的AUC大于KLF5 mRNA、TMEM64 mRNA,KLF5 mRNA+TMEM64 mRNA的AUC大于AFP+CEA+CA199。结论肝癌患者血清KLF5 mRNA、TMEM64 mRNA与AFP、CEA、CA199呈正相关,且KLF5 mRNA、TMEM64 mRNA联合诊断肝癌具有较高的价值。Objective To investigate the correlation between serum Krüpple-like factor 5(KLF5)mRNA and transmembrane protein 64(TMEM64)mRNA with alpha-fetoprotein(AFP),carcinoembryonic antigen(CEA),and car-bohydrate antigen 199(CA199)in patients with hepatocellular carcinoma(HCC),and to further analyze their diagnostic value.Methods A prospective study was conducted,and 170 HCC patients(HCC group),85 patients with benign liver diseases(benign group),and 85 healthy individuals(control group)admitted to Henan Cancer Hospital from January 2022 to August 2024 were selected in a 2:1:1 ratio.Serum levels of AFP,CEA,CA199,KLF5 mRNA,and TMEM64 mRNA were compared among the three groups.Pearson correlation analysis was used to assess the correlation between KLF5 mRNA,TMEM64 mRNA,and AFP,CEA,CA199.Univariate and multivariate logistic regression analyses were performed to identify influencing factors for HCC.Receiver operating characteristic(ROC)curves and the area under the curve(AUC)were used to evaluate the diagnostic value of AFP,CEA,CA199,KLF5 mRNA,TMEM64 mRNA,and their combinations(AFP+CEA+CA199 and KLF5 mRNA+TMEM64 mRNA)for HCC,and the predictive value of dif-ferent diagnostic schemes was compared.Results The levels of AFP,CEA,CA199,KLF5 mRNA,and TMEM64 mRNA in the HCC group were(35.82±11.93)ng/mL,(6.24±2.06)ng/mL,(58.40±19.33)ng/mL,8.86±2.94,and 6.95±2.30;in the benign group,the levels were(12.30±4.09)ng/mL,(3.59±1.19)ng/mL,(3.59±1.19)ng/mL,4.88±1.60,and 3.11±1.02;in the control group,the levels were(11.66±3.81)ng/mL,(3.25±1.08)ng/mL,(17.99±5.51)ng/mL,4.71±1.55,and 2.89±0.95;the levels of these indicators in the HCC group were significantly higher than those in the benign and con-trol groups(P<0.05),while there were no significant differences between the benign and control groups(P>0.05).Pear-son correlation analysis showed that KLF5 mRNA and TMEM64 mRNA were positively correlated with AFP,CEA,and CA199 in the HCC group(r=0.795,0.785,0.789;0.811,0.791,0.771,P<0.05).Multivariate logistic regression analysis in-dic
