AFP、CEA、CA724、CA199联合胃蛋白酶原检测对早期胃癌的诊断价值

摘　　要：目的:研究甲胎蛋白(AFP)、癌胚抗原(CEA)、癌抗原724(CA724)、癌抗原199(CA199)联合胃蛋白酶原(PG)检测对早期胃癌的诊断价值。方法:选取医院从2022年1月-2024年1月收治的80例早期胃癌患者开展研究,记作胃癌组。另取同期收治的80例良性胃病患者记作良性胃病组。检测并对比两组血清AFP、CEA、CA724、CA199与PG水平。以病理检查结果为“金标准”,分析血清肿瘤标志物联合PG诊断早期胃癌的效能。此外,对比不同临床分期胃癌患者AFP、CEA、CA724、CA199与PG水平。结果:胃癌组血清AFP、CEA、CA724、CA199水平均高于良性胃病组(均P<0.05)。胃癌组PG Ⅰ及PGI与PGI比值(PGR)低于良性胃病组(均P<0.05)。以病理检查结果为“金标准”,血清肿瘤标志物联合PG诊断早期胃癌的灵敏度、特异度及准确性均优于血清肿瘤标志物、PG单独诊断(均P<0.05)。Ⅰ-Ⅱ期胃癌患者AFP、CEA、CA724、CA199水平均低于Ⅲ-Ⅳ期患者,而PGⅠ、PGR水平均高于Ⅲ-Ⅳ期患者(均P<0.05)。结论:AFP、CEA、CA724、CA199联合PG检测在早期胃癌诊断中的价值较高,可显著提升灵敏度、特异度及准确性,有助于病情严重程度的评估。Objective:To study the diagnostic value of alpha-fetoprotein(AFP),carcinoembryonic antigen(CEA),cancer antigen 724(CA724),cancer antigen 199(CA199)combined with pepsinogen(PG)in early gastric cancer.Methods:80 patients with early gastric cancer admited to the hospital from January 2022 to January 2024 were studied and recorded as the gastric cancer group.In addition 80 patients with benign stomach disease admitted in the same period were recorded as benign stomach disease group.Serum levels of AFP,CEA,CA724,CA199 and PG were detected and compared between the two groups.Using pathological examination results as the"gold standard",the eficacy of serum tumor markers combined with PG in the diagnosis of early gastric cancer was analyzed.In addition,the levels of AFP,CEA,CA724,CA199 and PG in gastric cancer patients with different clinical stages were compared.Results:The levels of AFP,CEA,CA724 and CA199 in gastric cancer group were higher than those in benign gastric disease group(all P<0.05).The level of PⅠ and PⅠ to PⅡ ratio(PGR)in gastric cancer group was lower than that in benign gastric disease group(Ⅱ P<0.05)The sensitivity,specificity and accuracy of serum tumor marker combined with PG in the diagnosis of early gastric cancer were superior to that of serum tumor marker combined with PG alone(all P<0.05):.The levels ofAFP,CEA,CA724 and CA199 in stage Ⅰ to Ⅱ gastric cancer patients were lower than those in stage Ⅱ to Ⅳ patients,while the levels of PⅠ and PGR were higher than those in stage Ⅱ to Ⅳ patients(all P<0.05).Conclusion:AFP,CEA,CA724,CA199 combined with PG detection has high value in the diagnosis of early gastric cancer,which can significantly improve the sensitivity,specificity and accuracy,and contribute to the evaluation ofthe severity ofthe disease.
