摘 要:目的复发性流产(RSA)是常见的妊娠并发症之一。RSA病因复杂且具有异质性,其发病机制及防治方面仍有许多问题尚未得到解决,给临床治疗和预防带来极大的困难和挑战。近年来,多项研究报道RSA的发生与脂质代谢紊乱有关,RSA患者存在脂蛋白及脂质代谢异常,而且在RSA的早期防治中阿司匹林和低分子肝素(LMWH)除了抗凝和抗炎作用之外,还具有纠正脂质代谢紊乱的作用。本文旨在阐述RSA相关脂蛋白及脂质代谢异常的表现和具体机制,并对阿司匹林和LMWH在RSA防治中的作用进行综述,为RSA的防治提供新的思路和可行的策略。Objective Recurrent spontaneous abortion(RSA)represents a significant and prevalent pregnancy complication characterized by a multifaceted and heterogeneous etiology.Despite extensive research,the underlying mechanisms and strategies for prevention and management of RSA remain inadequately addressed,presenting considerable challenges in clinical practice.Recent studies have increasingly implicated that the pathogenesis of RSA is related to lipid metabolism dysregulation,RSA patients frequently exhibit abnormalities in lipoprotein and lipid metabolism.Besides their anticoagulant and anti-inflammatory effects,aspirin and low-molecular-weight heparin(LMWH)have also been shown to ameliorate lipid metabolism dysregulation in the early prevention and management of RSA.This review aims to elucidate the manifestations and specific mechanisms underlying lipid and lipid metabolite abnormalities associated with RSA,while reviewing the roles of aspirin and LMWH in the prevention and treatment of RSA,and to provide new ideas and feasible strategies for the prevention and treatment of RSA.
