摘 要:目的:探讨下肢康复机器人辅助步行功能训练联合中药口服对不完全脊髓损伤(SCI)恢复期患者下肢表面肌电信号及步行功能改善的临床疗效。方法:选择2023年3月—2023年12月于浙江康复医疗中心治疗的42例不完全SCI患者,随机将其分为A组和B组,各21例。A组采用补阳还五汤口服联合下肢康复机器人辅助步行功能训练治疗,B组采用常规步行功能训练治疗。两组均治疗12周。比较两组下肢表面肌电信号数据、步行速度、步行功能、运动功能及平衡能力。结果:治疗后,A组股直肌、胫骨前肌、腓肠肌表面肌电信号数据均高于B组,差异均有统计学意义(P<0.05)。治疗后,A组10米步行测试时间短于B组,差异有统计学意义(P<0.05)。治疗后,A组Holden步行功能分级情况优于B组,差异有统计学意义(P<0.05)。治疗后,A组美国SCI协会损伤量表中的运动功能评定项目评分高于B组,差异有统计学意义(P<0.05)。治疗后,A组Berg平衡量表评分高于B组,差异有统计学意义(P<0.05)。两组不良反应发生率分别为9.52%、0.00%,差异无统计学意义(P>0.05)。结论:补阳还五汤口服联合下肢康复机器人辅助步行功能训练能够改善不完全SCI患者下肢的行走能力,康复效果优于常规步行功能训练,具有应用价值。Objective:To investigate the clinical effect of lower limb rehabilitation robot assisted walking function training combined with orally traditional Chinese medicine on the improvement of lower limb surface electromyography signal and walking function in patients with incomplete spinal cord injury(SCI).Methods:A total of 42 patients with incomplete SCI treated in Zhejiang Rehabilitation Medical Center from March 2023 to January 2024 were selected,and they were randomly divided into group A and group B,with 21 cases each.Group A was treated with Buyang Huanwu decoction combined with lower limb rehabilitation robot assisted walking function training,group B was treated with conventional walking function training.Both groups were treated for 12 weeks.The surface EMG data,walking speed,walking function,motor function and balance ability of the two groups were compared.Results:After treatment,the surface EMG data of femoris rectus,tibialis anterior muscle and gastrocnemius muscle in group A were higher than those in group B,and the differences were statistically significant(P<0.05).After treatment,the 10-meter walking test time in group A was shorter than that in group B,and the difference was statistically significant(P<0.05).After treatment,Holden walking function classification in group A was better than that in group B,and the difference was statistically significant(P<0.05).After treatment,the scores of motor function assessment items in American SCI Association injury scale in group A were higher than those in group B,and the difference was statistically significant(P<0.05).After treatment,the Berg Balance Scale score of group A was higher than that of group B,and the difference was statistically significant(P<0.05).The incidence of adverse reactions between the two groups was 9.52%and 0.00%,respectively,with no statistical significance(P>0.05).Conclusion:Buyang Huanwu Decoction combined with lower limb rehabilitation robot assisted walking function training can improve the lower limb walking ability of patients
