经支气管Nd-YAG激光消融及冷冻联合化疗对中央型肺鳞癌治疗效果分析

摘　　要：目的评价经支气管Nd-YAG激光消融及冷冻联合化疗对中央型肺鳞癌的治疗效果。方法选择2018年1月31日-2023年12月31日在某院接受治疗的中央型肺鳞癌患者90例进行研究。按照排除标准排除8例后,通过随机数表法划分组别,依次为观察组及对照组各41例,对照组给予化疗,观察组予以经支气管Nd-YAG激光消融及冷冻联合化疗。对比两组疗效,肺功能指标,肿瘤标记物,以及FACT-L评分和不良反应。结果观察组的ORR为46.34%,DCR为92.68%,较对照组的24.39%、73.17%明显更高(P<0.05)。治疗后两组的FEV1/FVC、FEV1%及PEF较治疗前更高,而观察组较对照组更高(P<0.05)。治疗后两组的CEA、NSE及CA199较治疗前更低,而观察组较对照组更低(P<0.05)。治疗后两组的FACT-L各项评分较治疗前更高,而观察组较对照组更高(P<0.05)。观察组的不良反应总发生率是24.39%,较对照组34.15%差异无统计学意义(P>0.05)。结论经支气管Nd-YAG激光消融及冷冻联合化疗对CLSCC患者的作用效果较好,还可有效改善其肺功能及生活质量,值得推广。Objectives This study aims to evaluate the effects of transbronchial Nd-YAG laser ablation and cryotherapy combined with chemotherapy in the treatment of central lung squamous cell carcinoma.Methods 90 patients with CLSCC treated in hospital from January 31,2018 to December 30,2023 were selected for the study.After the exclusion of 8 cases according to the exclusion criteria,the patients were divided by random number table,with 41 cases in the observation group and 41 cases in the control group.Chemotherapy was given to the control group,and transbronchial Nd-YAG laser ablation and cryocombination chemotherapy were given to the observation group.Efficacy,lung function measures,tumor markers,FACT-L scores and adverse reactions were compared.Results The ORR and DCR of the observation group were 46.34%and 92.68%,which were significantly higher than those of 24.39%and 73.17%in the control group(P<0.05).After treatment,FEV1/FVC,FEV1%and PEF in both groups were higher than those before treatment,and those in the observation group were higher than those in the control group(P<0.05).CEA,NSE and CA199 after treatment in both groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).FACT-L scores were higher than before treatment and higher in the observation group than in the control group(P<0.05).The overall incidence of adverse reactions in the observation group was 24.39%,with no significant difference from 34.15%in the control group(P>0.05).Conclusions Transbronchial Nd-YAG laser ablation and cryocombination chemotherapy have good effects and high safety on CLSCC patients,and can also effectively improve their lung function and quality of life,which is worth promoting.
