健康信念在中青年脑卒中患者复发风险感知与功能锻炼依从性间的中介效应

摘　　要：目的 探讨中青年脑卒中患者健康信念、复发风险感知与功能锻炼依从性现状,明确三变量间的关系,并分析健康信念在中青年脑卒中患者复发风险感知与功能锻炼间的中介效应。方法 采用一般资料调查表、脑卒中健康信念量表、脑卒中患者复发风险评估量表和脑卒中患者功能锻炼依从性量表对286例中青年脑卒中患者进行调查。结果 286名中青年脑卒中患者脑卒中健康信念、复发风险感知与功能锻炼依从性总分分别为(126.59±15.8)分、(49.97±8.62)分、(40.89±5.89)分。患者复发风险感知与健康信念、功能锻炼依从性呈正相关(r 1=0.470,r 2=0.402,P<0.01),健康信念与功能锻炼依从性呈正相关(r 3=0.628,P<0.01);健康信念在复发风险感知与功能锻炼依从性之间起部分中介作用,中介效应占总效应的45.5%。结论 中青年脑卒中患者功能锻炼依从性处于中等水平,患者脑卒中复发风险感知既可以直接影响患者功能锻炼的依从性,也可以通过影响患者的健康信念继而影响其功能锻炼的依从性。Objective To investigate the status quo of health belief,recurrence risk perception and functional exercise compliance in middle-aged and young stroke patients,identify the relationship between the three variables,and analyze the mediating effect of health belief on patients’recurrence risk perception and functional exercise.Methods A total of 286 middle-aged and young stroke patients were investigated by general data questionnaire,stroke health belief Scale,stroke patients recurrence risk assessment scale and stroke patients functional exercise compliance scale.Results The total scores of stroke health belief,recurrence risk perception and functional exercise compliance in 286 middle-aged and young stroke patients were(126.59±15.8),(49.97±8.62)and(40.89±5.89)points,respectively.Patients’recurrence risk perception was positively correlated with health belief and functional exercise compliance(r 1=0.470,r 2=0.402,P<0.01),and health belief was positively correlated with functional exercise compliance(r 3=0.628,P<0.01).Health belief play a partial mediating role between relapse risk perception and functional exercise compliance,and the mediating effect accounted for 45.5%of the total effect.Conclusion The functional exercise compliance of middle-aged and young stroke patients is at a moderate level.The perceived risk of stroke recurrence can directly affect the functional exercise compliance of patients,and can also affect the functional exercise compliance of patients through affecting their health beliefs.
