青年脑卒中患者及配偶的生育意愿与需求的质性研究

摘　　要：初育年龄“延迟化”及脑卒中患病年龄“提前化”,导致青年卒中群体的生育潜力可能没有完全释放,亟须管理部门的重视。文章采用目的抽样法选择安徽省某三级甲等医院神经内科诊治的15例青年卒中患者及其配偶进行半结构式深入访谈,应用Colaizzi现象学分析法分析原始资料,提炼出四个主题:生育意愿普遍较低,担心疾病对生育的不良影响,缺乏脑卒中康复及生育安全相关教育,需要心理、社会及家庭支持。由此提出综合干预策略:加强医疗支持与健康指导(普及健康知识教育、设立专项医疗咨询门诊、强化康复与随访服务),改善社会环境与支持体系。Despite the increasingly evident trend of“delayed age”of first childbearing,the advancing onset age of stroke has prevented the full realization of reproductive potential among young stroke groups.The continued decline in fertility intentions of young chronic disease patients,especially those with stroke,requires immediate attention from relevant administrative authorities.This study used purposive sampling to select 15 young stroke patients and spouses receiving treatment in the neurology department of a tertiary hospital in Anhui province for semi-structured,in-depth interviews.Using Colaizzi’s descriptive phenomenological method,four major themes were identified from the original data:generally low fertility intentions,concerns about the negative impact of illness on reproduction,a lack of education related to stroke rehabilitation and fertility safety,and the need for psychological,social,and family support.This study concludes that the fertility intentions of young stroke patients are generally low,with influencing factors mainly including individual factors,psychological and social factors,and economic and environmental factors.To address the identified needs,this study proposed comprehensive intervention strategies,including enhancing medical support and health guidance by promoting health education,establishing specialized medical consultations,and enhancing rehabilitation and follow-up services,as well as improving the social environment and support system.
