摘 要:目的探讨微型种植体支抗在口腔正畸治疗中的应用效果。方法队列研究。抽取2020年1月至2022年1月榆林市第一医院收治的80例口腔正畸治疗患者,按照随机数字表法分为对照组和观察组,每组40例。对照组接受传统正畸治疗(使用金属托槽和弓丝系统矫治),观察组在此基础上采用微型种植体支抗治疗。比较两组正畸效果、颞下颌关节间隙变化、牙龈情况及咀嚼功能。结果观察组矫正时间、拔牙间隙闭合时间、磨牙移位、上中切牙倾斜差及上中切牙凸距均小于对照组(P均<0.05);两组矫治前后颞下颌关节前、上、后、内、外间隙比较差异未见统计学意义(P>0.05);观察组牙龈菌斑指数、牙龈出血指数以及牙龈指数均低于对照组(P均<0.05);观察组咀嚼咬合力与咀嚼效率均高于对照组(P均<0.05)。结论与传统方法相比,微型种植体支抗在口腔正畸治疗中不仅缩短了治疗时间,提高了治疗效果,改善了牙龈状况和咀嚼功能,且对颞下颌关节无负面影响,是一种有效的辅助治疗手段。ObjectiveTo investigate the application efficacy of micro-implant anchorage during orthodontics.MethodsEighty patients treated by orthodontic treatment in Yulin First Hospital from January 2020 to January 2022 were selected for the cohort study.And they were divided into a control group and an observation group using a random number table method,with 40 cases in each group.The control group received traditional orthodontic treatment(metal bracket and archwire system),and the observation group was treated by micro-implant anchorage based on metal bracket and archwire system.The orthodontic effect,temporomandibular joint space changes,gingival conditions and masticatory function of the two groups were compared.ResultsThe correction time,closure time of extraction space,molar displacement,tilt difference of upper central incisor and convex distance difference of upper central incisor in the observation group were significantly shorter or lower than those in the control group(all P<0.05).There were no obvious changes in the anterior,upper,post,internal and external spaces of temporomandibular joint between the two groups before and after correction(P>0.05).The gingival plaque index,gingival bleeding index and gingival index in the observation group were significantly lower than those in the control group(all P<0.05).The masticatory force and masticatory efficiency in the observation group were significantly higher compared with the control group(all P<0.05).ConclusionsCompared with traditional method,micro-implant anchorage during orthodontics can not only shorten the treatment time,enhance the therapeutic effect,improve the gingival conditions and masticatory function but also has no negative impact on the temporomandibular joint.Moreover,it can,considering as an effective adjuvant therapy.
