摘　　要：目的系统评价母亲糖尿病与子代唇腭裂发生的相关性。方法计算机检索PubMed、Embase、Cochrane Library、Web of Science、CNKI、VIP及WanFang Data数据库,搜集有关母亲糖尿病和子代唇腭裂相关性的观察性研究,检索时限均为建库至2024年6月30日。由2名研究员独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Stata 17.0和RevMan 5.4软件进行Meta分析。结果共纳入34项研究,包括2556911名研究对象。Meta分析结果显示:母亲孕前患糖尿病[OR=1.91,95%CI(1.59,2.30),P<0.01],包括1型糖尿病、2型糖尿病[T1DM:OR=2.28,95%CI(1.65,3.16),P<0.01;T2DM:OR=1.87,95%CI(1.10,3.19),P<0.01]均会增加子代唇腭裂风险。但母亲妊娠期糖尿病不会增加子代唇腭裂发生风险。结论当前证据显示,母亲孕前患糖尿病与子代唇腭裂发生增加相关。受纳入研究数量和质量的限制,上述结论尚待更多高质量研究予以验证。Objective To systematically review the associations of maternal diabetes mellitus with cleft lip and palate in offspring.Methods The PubMed,Embase,Cochrane Library,Web of Science,CNKI,VIP and WanFang Data databases were electronically searched to collect the observational studies on the association of maternal diabetes mellitus with cleft lip and palate in offspring from inception to June 30,2024.Two reviewers independently screened literature,extracted data and assessed the risk of bias of the included studies.Meta-analysis was then performed by using Stata 17.0 and RevMan 5.4 software.Results A total of 34 studies involving 2556911 participants were included.The results of meta-analysis showed that maternal pregestational diabetes mellitus was associated with an increased risk of cleft lip and palate in offspring(OR=1.91,95%CI 1.59 to 2.30,P<0.01).Maternal type 1 diabetes mellitus(T1DM)and type 2 diabetes mellitus(T2DM)were also associated with an increased risk of cleft lip and palate in offspring(T1DM:OR=2.28,95%CI 1.65 to 2.30,P<0.01;T2DM:OR=1.87,95%CI 1.10 to 3.19,P<0.01).There was no correlation between maternal gestational diabetes mellitus and cleft lip and palate in offspring.Conclusion Current evidence shows that maternal pregestational diabetes mellitus was associated with an increased incidence of cleft lip and palate in offspring.Due to the limited quality and quantity of the included studies,more high quality studies are needed to verify the above conclusion.
