老年高血压合并急性脑梗死患者血清Lp-PLA2、Hcy水平与病情、疾病转归的关系

摘　　要：目的:分析老年高血压合并急性脑梗死(ACI)患者血清脂蛋白相关磷脂酶A2(Lp-PLA2)、同型半胱氨酸(Hcy)水平与病情、疾病转归的关系。方法:将107例老年高血压合并ACI患者纳入观察组;同期纳入100名健康体检者为对照组。将观察组患者依据病情、疾病转归分组,分析血清Lp-PLA2、Hcy水平与患者病情及疾病转归的关系。结果:观察组Lp-PLA2、Hcy水平高于对照组(P<0.05);重度组患者Lp-PLA2、Hcy水平高于中度组,中度组高于轻度组(P<0.05);转归不佳组患者Lp-PLA2、Hcy水平高于转归良好组(P<0.05);血清Lp-PLA2、Hcy水平与病情中重度、疾病转归不佳正相关(P<0.05);血清Lp-PLA2、Hcy水平单独检测及联合检测对病情评估的曲线下面积(AUC)分别为0.786、0.861、0.916,对疾病转归预测的AUC分别为0.767、0.667、0.801。结论:老年高血压合并ACI患者血清Lp-PLA2、Hcy水平升高,且上述指标可良好预测评估患者病情及疾病转归。Objective:To analyze the relationship between serum lipoprotein-associated phospholipase A2(Lp-PLA2)and homocysteine(Hcy)levels and condition,outcomes in elderly patients with hypertension and acute cerebral infarction(ACI).Methods:A total of 107 elderly patients with hypertension and ACI were selected as the observation group.Meanwhile,100 healthy individuals were selected as the control group.Patients in the observation group were grouped according to the severity and outcome of the disease.The relationship between serum Lp-PLA2,Hcy levels and the severity and outcome of the disease was analyzed.Results:Compared with control group,the levels of Lp-PLA2 and Hcy were higher in the observation group(P<0.05).The levels of Lp-PLA2 and Hcy in the severe group,the moderate group,and the mild group decreased in order(P<0.05).The levels of Lp-PLA2 and Hcy in patients with poor outcomes were higher than those in patients with good outcomes(P<0.05).Serum Lp-PLA2 and Hcy levels were positively correlated with the severity and outcome of the disease(P<0.05).The AUC s of serum Lp-PLA2,Hcy,and their combination for evaluating moderate to severe condition were 0.786,0.861,and 0.916.The AUC s for predicting disease outcomes were 0.767,0.667,and 0.801.Conclusion:Serum Lp-PLA2 and Hcy levels in elderly patients with hypertension and ACI are elevated.The two can well help to evaluate the patient's condition and predict disease outcome.
