摘 要:目的:分析乳腺超声S-Detect(人工智能)技术在BI-RADS 4级乳腺肿块诊断中的应用价值。方法:回顾性分析102例经常规超声诊断为BI-RADS 4级乳腺肿块(102个肿块)患者的临床资料,患者均行乳腺超声S-Detect技术检测,以组织病理结果为金标准,比较常规灰阶超声、乳腺超声S-Detect技术对BI-RADS 4级乳腺肿块的诊断效能。结果:102例BI-RADS 4级乳腺肿块有62例经病理证实,其中良性肿块33例;恶性肿块29例。乳腺超声S-Detect技术诊断BI-RADS 4级乳腺良恶性肿块的敏感度、特异度、准确率、阳性和阴性预测值为93.10%、96.97%、95.16%、96.43%和96.12%,高于常规超声的72.41%、78.79%、75.81%、75.00%和76.47%(P<0.05)。受试者工作特征(ROC)曲线分析显示,乳腺超声S-Detect技术诊断BI-RADS 4级乳腺良恶性肿块的曲线下面积(AUC)为0.847,大于常规超声的0.718(Z=2.274,P=0.023)。结论:乳腺超声S-Detect技术鉴别诊断BI-RADS 4级乳腺良恶性肿块的价值较高,值得临床推广。Objective:To analyze the application value of breast ultrasound artificial intelligence S-Detect technology in the diagnosis of BI-RADS 4 breast masses.Methods:Totally 102 patients diagnosed with BI-RADS 4 breast masses(102 masses)by conventional ultrasound were reviewed.All of them underwent detection using breast ultrasound S-Detect technology.Histopathological results were used as the gold standard to compare the diagnostic efficacy of conventional ultrasound and breast ultrasound S-Detect technology for BI-RADS 4 breast masses.Results:Among the 102 cases enrolled,62 cases were confirmed by pathology,including 33 cases with benign masses and 29 cases with malignant masses.The sensitivity,specificity,accuracy,positive and negative predictive values of breast ultrasound S-Detect technology for diagnosing benign and malignant BI-RADS 4 breast masses(93.10%,96.97%,95.16%,96.43%and 96.12%)were higher than those of conventional ultrasound(72.41%,78.79%,75.81%,75.00%and 76.47%)(P<0.05).The AUC of breast ultrasound S-Detect technology was 0.874,which was larger than that of conventional ultrasound(0.718)(Z=2.274,P=0.023).Conclusion:Breast ultrasound S-Detect technology has high value in differential diagnosis of benign and malignant BI-RADS 4 breast masse and is worthy of clinical promotion.
