血清VPN与急性脑梗死患者病情进展及预后的相关性分析

摘  要:目的观察血清内脏脂肪型丝氨酸蛋白酶抑制剂(vaspin,VPN)与急性脑梗死患者病情进展及预后的关联。方法本研究为回顾性研究,研究对象为郑州大学第二附属医院2022年1月至2023年6月收治的96例急性脑梗死患者,依据病情严重程度不同将其分别列为轻度组(40例)、中重度组(56例),比较两组患者的炎症指标,神经功能血清指标以及VPN间的差异;经Pearson相关性系数检验血清VPN与炎症指标、神经功能血清指标的相关性;统计入组患者不良预后发生情况,比较预后不良组、预后良好组患者的改良Rakin指数(mRs)、改良Barthel指数(MBI)、VPN间的差异,验证血清VPN对急性脑梗死不良预后的预测价值。结果中重度组的高迁移率族蛋白1(HMGB1)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平均高于轻度组(P<0.05)。中重度组的神经元轴突轻链(NF-L)、神经元特异性烯醇化酶(NSE)、中枢神经特异蛋白(S100β)水平均高于轻度组,VPN水平低于轻度组(P<0.05)。血清VPN水平与HMGB1、hs-CRP、TNF-α、IL-6、NF-L、NSE、S100β水平负相关(P<0.05)。经统计,96例急性脑梗死患者的不良预后发生率为16.67%(16/96);预后不良组的VPN水平、MBI评分均低于预后良好组,预后不良组的mRs评分高于预后良好组(P<0.05)。血清VPN水平和mRs、MBI联合检测对急性脑梗死不良预后的预测灵敏度、特异度均高于VPN单独检测、mRs单独检测、MBI单独检测。结论血清VPN水平与急性脑梗死病情严重程度负相关,联合检测VPN、mRs、MBI能实现对患者不良预后的早期预测。Objective To identify the correlation of serum vaspin(VPN)with the progression and prognosis of acute cerebral infarction(ACI).Methods This was a retrospective study involving 96 patients with ACI admitted to the Second Affiliated Hospital of Zhengzhou University from January 2022 to June 2023.They were classified into mild group(40 cases)and moderate-to-severe group(56 cases)according to the severity of their condition.The differences in inflammation indicators,serum indicators of neurological function,and VPN between the two groups of patients were compared.Pearson correlation coefficient was used to test the correlation of serum VPN with inflammatory and neurological function serum indicators.The occurrence of poor prognosis in enrolled patients was recorded.The modified Rankin scale(mRs),modified Barthel index(MBI)and VPN between patients with poor and good prognosis were compared to verify the predictive value of serum VPN for poor prognosis of ACI.Results High mobility group protein box-1(HMGB1),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the moderate-to-severe group were significantly higher than those of the mild group(P<0.05).The levels of neurofilament light chain(NF-L),neuron-specific enolase(NSE)and central nerve S100-βprotein(S100β)in the moderate-to-severe group were significantly higher than those of the mild group,and the VPN level was significantly lower(P<0.05).Serum VPN was negatively correlated with HMGB1,hs-CRP,TNF-α,IL-6,NF-L,NSE and S100β(P<0.05).The incidence of poor prognosis in 96 patients with ACI was 16.67%(16/96).The VPN and MBI of the poor prognosis group were significantly lower than those of the good prognosis group,and the mRs of the poor prognosis group was significantly higher(P<0.05).The sensitivity and specificity of the combined detection of serum VPN,mRs and MBI in predicting the poor prognosis of ACI were significantly higher than those of VPN,mRs and MBI alone.Conclusion Serum VPN is negatively correlated with t
