复方樟柳碱联合曲伏前列素治疗开角型青光眼的效果分析

摘　　要：目的分析复方樟柳碱注射液联合曲伏前列素治疗开角型青光眼患者的效果。方法按照随机数字表法将92例(包含159只眼)开角型青光眼患者分为两组。对照组给予曲伏前列素治疗,观察组在对照组的基础上给予复方樟柳碱注射液治疗。对比两组疗效、不良反应发生率、视力、眼压、视野缺损、基质金属蛋白酶抑制因子-2(TIMP-2)、内皮素-1(EF-1)、基质金属蛋白酶(MMP)、视觉相关生活质量(VRQL)评分,以及视网膜、眼动脉的阻力指数(RI)及收缩期血流峰值速度(PSV)的分别测得值。结果观察组总有效率为93.83%(76/81),高于对照组的82.05%(64/78),差异具有统计学意义(P<0.05);治疗后,观察组视野缺损的测得值、眼压、EF-1水平、TIMP-2水平、MMP水平、视网膜及眼动脉的RI均低于对照组,VRQL评分、视网膜与眼动脉的PSV及视力测得值均高于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论曲伏前列素与复方樟柳碱注射液联合治疗开角型青光眼患者的效果确切,安全性较好,并可调节眼压及眼部血液循环,改善患者视力水平,提升生活质量。Objective To analyze the effect of compound Anisodine Hydrobromide injection combined with Travoprost on patients with open-angle glaucoma.Methods According to the random number table method,92 patients(including 159 eyes)with open-angle glaucoma were divided into two groups.The control group were treated with clopidogrel,while the observation group were received compound Anisodine Hydrobromide injection in addition to clopidogrel.The efficacy,incidence of adverse reactions,visual acuity,intraocular pressure,visual field defects,matrix metalloproteinase inhibitor-2(TIMP-2),endothelin-1(ET-1),matrix metalloproteinase(MMP),visual related quality of life(VRQL)score,resistance index(RI)of the retina and ophthalmic artery,and peak systolic blood flow velocity(PSV)were compared between the two groups.Results The total effective rate in the observation group was 93.83%(76/81),which was higher than the 82.05%(64/78)in the control group,and the difference was statistically significant(P<0.05).After treatment,the values for visual field defects,intraocular pressure,ET-1 level,TIMP-2 level,MMP level,and RI of the retina and ophthalmic artery in the observation group were lower than those in the control group.Conversely,the VRQL score,PSV of the retina and ophthalmic artery,and visual acuity were higher in the observation group compared to the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination therapy of Anisodine Hydrobromide injection and Travoprost has a definite effect and good safety in the treatment of patients with open-angle glaucoma.It can also regulate intraocular pressure and ocular blood circulation,improve patients′visual acuity,and enhance their quality of life.
