疏肝润目汤联合环孢素滴眼液治疗干眼症疗效观察

摘　　要：目的分析疏肝润目汤联合环孢素滴眼液治疗干眼症的效果。方法回顾性分析登封嵩正中医院94例干眼症患者资料,根据治疗方法将采用环孢素滴眼液治疗的47例患者列为对照组,将采用疏肝润目汤+环孢素滴眼液治疗的47例患者列为研究组。比较两组临床疗效、中医证候积分、角膜荧光素染色(FL)积分、干眼症相关指标[泪液分泌试验(SIT)、角膜表面不规则指数(SAI)、泪膜破裂时间(BUT)]、结膜上皮细胞中炎症因子[白细胞介素-1β(IL-1β)、细胞间黏附分子-1(ICAM-1)、肿瘤坏死因子-α(TNF-α)]。结果研究组临床总有效率95.74%(45/47)高于对照组82.98%(39/47),差异有统计学意义(P<0.05);研究组治疗后中医证候积分、FL积分低于对照组(P<0.05);研究组治疗后SAI、IL-1β、ICAM-1、TNF-α低于对照组,SIT、BUT高于对照组(P<0.05)。结论疏肝润目汤辅助环孢素滴眼液治疗干眼症患者疗效显著,有助于减轻患者中医证候,改善基础泪液分泌量,延长BUT。Objective To analyze the effect of Shugan Runmu decoction combined with cyclosporine eye drops on the dry eye syndrome.Methods Retrospective analysis of the data of 94 patients with dry eye syndrome in Dengfeng Songzhong Hospital was completed.According to the treatment method,47 patients in the control group were treated with cyclosporine eye drops,and 47 patients in the study group were treated with Shugan Runmu Decoction and cyclosporine eye drops.The clinical efficacy,TCM syndrome score,corneal fluorescein staining(FL)score,dry eyerelated indicators[tear secretion test(SIT),corneal surface irregularity index(SAI),tear film breakup time(BUT)],and the levels of inflammatory factors in conjunctival epithelial cells[interleukin-1β(IL-1β),intercellular adhesion molecule-1(ICAM-1),tumor necrosis factor-α(TNF-α)]were compared between the two groups.Results The total clinical effective rate in the study group was 95.74%(45/47),which was higher than 82.98%(39/47)in the control group(P<0.05).After treatment,the TCM syndrome score and FL score in the study group were lower than those in the control group(P<0.05).After treatment,the levels of SAI,IL-1β,ICAM-1,and TNF-αin the study group were lower than those in the control group,while the levels of SIT and BUT were higher than those in the control group(P<0.05).Conclusion Shugan Runmu decoction combined with Cyclosporine eye drops has significant curative effect in the patients with dry eye,which helps to reduce the TCM syndrome of patients,improve the basic tear secretion,and prolong the BUT.
