摘 要:目的分析微小RNA(miR-214)与脑梗死患者神经功能缺损程度的关联,以及对继发性癫痫(SE)发生风险的预测价值。方法选取102例脑梗死患者分为轻度组(40例)、中重度组(56例),检验miR-214与神经功能指标、神经功能评分的相关性及对脑梗死患者SE发生风险的预测价值。结果经检测,中重度组的miR-214低于轻度组,NF-L、NSE、S100β、mRs评分均高于轻度组,中重度组的MBI评分低于轻度组,差异有统计学意义(P<0.05)。经Pearson相关性系数检验,miR-214与NF-L、NSE、S100β、mRs负相关,与MBI正相关(P<0.05)。经统计,102例脑梗死患者的SE发生率为21.57%(22/102);SE组的miR-214、MBI评分均低于非SE组,mRs评分高于非SE组(P<0.05)。经绘制受试者工作特征曲线(ROC)验证,血清miR-214、mRs、MBI联合检测对脑梗死SE发生风险的预测灵敏度、特异度均高于miR-214单独检测、mRs单独检测、MBI单独检测。结论miR-214水平与脑梗死神经功能缺损程度负相关,联合检测miR-214、mRs、MBI能实现对患者SE发生风险的早期预测。Objective To analyze the association between miR-214 and the degree of neurological deficit and to analyze its predictive value for the risk of secondary epilepsy(SE)in the patients with cerebral infarction.Methods 102 patients with cerebral infarction were selected and divided into the mild group(40 cases)and the moderate to severe group(56 cases).The correlation between miR-214 and the neurological function indicators,neurological function scores and its predictive value for the risk of SE were analyzed in the patients with cerebral infarction.Result The miR-214 level in the moderate to severe group was lower than that in the mild group,while the levels of NF-L,NSE,S100β,and mRs scores were higher than those in the mild group.MBI scores in the moderate to severe group were lower than those in the mild group,showing a statistically significant difference(P<0.05).According to Pearson correlation coefficient test,the miR-214 level was negatively correlated with the levels of NF-L,NSE,S100β,and mRs,and was positively correlated with the MBI(P<0.05).According to statistics,the incidence of SE in 102 patients with cerebral infarction was 21.57%(22/102).The miR-214 and MBI scores in the SE group were lower than those in the non SE group,and the mRs score was higher than that on the non SE group(P<0.05).Verified by drawing receiver operating characteristic(ROC)curves,the combined detection of serum miR-214,mRs,and MBI showed higher sensitivity and specificity in predicting the risk of stroke SE compared to miR-214 alone,mRs alone,and MBI alone.Conclusion The level of miR-214 is negatively correlated with the degree of neurological deficit in the patients with cerebral infarction.Combined detection of miR-214,mRs,and MBI achieves early prediction of the risk of SE in patients with cerebral infarction.
