摘 要:目的探索体重调整腰围指数(WWI)对儿童青少年骨密度(BMD)的影响,以及是否存在饱和效应。方法采用NHANES数据库2011-2018年的健康数据,使用多变量线性回归模型评估WWI与BMD之间的关系。通过RCS回归曲线分析饱和效应来确定非线性关系和饱和值。同时,还进行了亚组分析。结果本研究纳入了2777名12~20岁的儿童青少年。我们发现WWI与总BMD显著负相关(β=-0.06,95%CI:(-0.07~-0.06),P<0.001),在按年龄、性别分层的亚组中仍然呈显著负相关(P<0.001)。WWI和BMD具有特定的饱和效应,总人群中WWI与BMD之间的饱和效应值为9.22 cm/kg^(0.5),当WWI低于9.22 cm/kg^(0.5)时,效应值为-0.24。然而,当BMI超过9.22 cm/kg^(0.5)时,效应值变为-0.17。结论WWI与儿童青少年的总BMD之间存在显著的负相关关系和饱和效应值。Objective To explore the effect of weight-adjusted waist circumference index(WWI)on bone mineral density(BMD)in children and adolescents and whether there is a saturation effect.Methods The relationship between WWI and BMD was assessed using health data from the NHANES database from 2011-2018,with a multivariate linear regression model.Non-linear relationships and saturation values were determined by analyzing saturation effects through RCS regression curves.Subgroup analyses was also performed.Results This study included 2777 children and adolescents aged from 12 to 20 years old.We found that WWI was significantly and negatively associated with total BMD(β=-0.06,95%CI:-0.07--0.06,P<0.001),and remained significant in subgroups stratified by age and sex(P<0.001).WWI and BMD had a specific saturation effect,with a saturation effect value of 9.22 cm/kg^(0.5) between WWI and BMD in the total population.When WWI was lower than 9.22 cm/kg^(0.5),the effect value was-0.24.However,when BMI exceeded 9.22 cm/kg^(0.5),the effect value became-0.17.Conclusion Our study demonstrate a significant negative association and saturation effect value between WWI and total BMD in children and adolescents.
