摘 要:在肿瘤外科治疗中,手术根治程度是决定患者预后的关键因素,经典R0切除一直是胃癌手术的核心目标。然而,精准外科时代的到来,基因组学、蛋白质组学等技术以及大数据和人工智能的应用,推动了胃癌治疗模式从“经验驱动”向“数字驱动”的变革,沿用了近50年的R_(0)切除概念面临新的挑战。笔者结合相关临床研究,重新审视并展望在人工智能赋能精准外科时代,临床经典R_(0)切除概念如何适应分子病理学诊断、影像分析、人工智能辅助决策、围手术期综合治疗等领域最新研究进展。In the surgical oncology,the prognosis of patients is mainly dependent on the surgical radicality,with conventional R_(0) resection long regarded as the cornerstone of gastric cancer.However,under the precision surgical era,technologies such as genomics,proteomics,and the application of big data and artificial intelligence(AI)have driven the transformation of gastric cancer treatment from the"experience-driven"to the"data‐driven"model.This thus poses novel challenges to the conventional R_(0) resection,which has prevailed for nearly five decades.The authors combine relevant clinical research to review and anticipate how the classic concept of R_(0) resection can adapt to the latest research advancements in molecular pathological diagnosis,imaging analysis,AI-assisted decision-making,and perioperative comprehensive treatment under the AI empowered precision surgical era.
