地夸磷索钠滴眼液治疗角膜移植术后干眼的疗效

摘　　要：目的观察地夸磷索钠滴眼液治疗角膜移植术后干眼的临床疗效。方法48例(48眼)角膜移植术后干眼的患者根据不同治疗方式分为两组。对照组25例(25眼)使用0.3%玻璃酸钠滴眼液治疗;试验组23例(23眼)使用3%地夸磷索钠滴眼液联合0.3%玻璃酸钠滴眼液治疗。比较两组治疗前(T0)、治疗3周(T1)和治疗7周(T2)时的眼表疾病指数(OSDI)评分、泪膜破裂时间(BUT)、基础泪液分泌试验Ⅰ(SⅠt)和角膜荧光染色(CFS)评分,评估两组患者的临床疗效,记录不良反应发生情况。结果T0、T1时,两组间OSDI评分、BUT、SⅠt和CFS评分比较均无统计学差异(P>0.05)。T1时,试验组CFS评分低于T0时(P<0.05)。T2时,对照组BUT大于T0时,CFS评分低于T0和T1时(P<0.05)。T2时,试验组OSDI评分和CFS评分均低于T0和T1时,BUT和SⅠt均大于T0和T1时(P<0.05),且OSDI评分和CFS评分低于对照组,BUT和SⅠt大于对照组(P<0.05)。试验组总有效率高于对照组(87.0%vs.60.0%)(P<0.05)。两组不良反应发生率比较无统计学差异(P>0.05)。结论地夸磷索钠滴眼液治疗角膜移植术后干眼疗效确切,安全性高,但其眼表改善作用需要较长的时间。Objective To observe the clinical efficacy of diquafosol sodium eye drops in the treatment of dry eye after keratoplasty.Methods Forty-eight patients with dry eye(48 eyes)after keratoplasty were divided into two groups according to different treatment methods.Group C(25 eyes)was treated with 0.3%sodium hyaluronate eye drops and group A(23 eyes)was treated with a combination of 3%diquafosol sodium eye drops and 0.3%sodium hyaluronate eye drops.The ocular surface disease index(OSDI)score,tear film break-up time(BUT),basic tear secretion testⅠ(SⅠt)and corneal fluorescence staining(CFS)score were compared between the two groups before dry eye treatment(T0),at 3 weeks(T1)and 7 weeks of treatment(T2).The clinical efficacy was evaluated and the occurrence of adverse reaction was recorded.Results There were no significant differences in OSDI score,BUT,SⅠt and CFS score between the two groups at T0 and T1(P>0.05).The CFS score at T1 was lower than that at T0 in group A(P<0.05).The BUT at T2 was higher than that at T0,and the CFS score at T2 was lower than that at T0 and T1 in group C(P<0.05).The OSDI and CFS scores were lower,while the BUT and SⅠt were higher at T2 than those at T0 and T1 in group A(P<0.05).Additionally,the OSDI and CFS scores in group A were lower,while the BUT and SⅠt were higher than those in group C at T2(P<0.05).The total effective rate of group A was higher than that of group C(87.0%vs.60.0%)(P<0.05).There was no significant difference in the incidence of adverse reaction between the two groups(P>0.05).Conclusion Diquafosol sodium eye drops has a definite therapeutic effect and high safety in treating dry eye after keratoplasty,but the ocular surface improvement effect requires a long time.
