绝经后子宫内膜息肉经宫腔镜手术切除后复发危险因素探讨

摘　　要：目的探讨绝经后子宫内膜息肉经宫腔镜手术切除后复发危险因素。方法回顾性纳入2022年1月至2023年1月于苍南县妇幼保健院接受宫腔镜手术切除治疗绝经后子宫内膜息肉患者共100例,根据术后随访1年是否复发分组,采用单因素和多因素评估患者手术切除后复发危险因素。结果本研究纳入100例患者手术切除后随访1年复发共7例,复发率为7.00%。单因素分析结果显示,绝经前有无子宫内膜息肉病史、是否为腺肌瘤样息肉及内膜病理组织学类型均与绝经后子宫内膜息肉经宫腔镜手术切除后复发有关(P<0.05);多因素分析结果显示,绝经前子宫内膜息肉病史、腺肌瘤样息肉及合并增殖期内膜均是绝经后子宫内膜息肉经宫腔镜手术切除后复发独立危险因素(P<0.05)。结论绝经后子宫内膜息肉经宫腔镜手术切除后复发风险与绝经前有无子宫内膜息肉病史、息肉及内膜病理组织学类型密切相关。Objective To investigate the risk factors for recurrence of patients with postmenopausal endometrial polyps after hysteroscopic resection.Methods 100 patients with postmenopausal endometrial polyps underwent hysteroscopic surgical resection in the hospitals of our county from January 2022 to January 2023 were retrospectively included and were grouped according to whether recurred after 1 years of postoperative follow-up or not,and the risk factors for recurrence after surgical resection were evaluated by univariate and multivariate methods.Results There were 7 patients with recurrences in 1 years after surgical resection,and the recurrence rate was 7.00%.Univariate analysis showed that premenopausal history of endometrial polyps,adenomyomatoid polyps and endometrial histopathological types were related to the recurrence of postmenopausal endometrial polyps after hysteroscopic resection(P<0.05).Multivariate analysis showed that premenopausal history of endometrial polyps,adenomyomatoid polyps and proliferative membranes were independent risk factors for recurrence of postmenopausal endometrial polyps after hysteroscopic surgery(P<0.05).Conclusion The recurrence risk of postmenopausal endometrial polyps after hysteroscopic resection is closely related to the presence or absence of premenopausal endometrial polyp history,polyp and endometrial histopathological types.
