摘 要:目的目的探讨糖类抗原19-9(CA19-9)、甲胎蛋白(AFP)、神经元特异性烯醇化酶(NSE)在消化道恶性肿瘤诊断中的临床应用价值。方法方法选取60例消化道恶性肿瘤患者作为肿瘤组,36例健康者作为对照组,比较两组受试者血清CA19-9、AFP、NSE水平及不同消化道恶性肿瘤患者血清CA19-9、AFP、NSE水平。采用受试者工作特征(ROC)曲线及曲线下面积(AUC)分析CA19-9、AFP、NSE单独及联合检测对消化道恶性肿瘤的诊断价值。结果结果肿瘤组患者血清CA19-9、AFP、NSE水平均明显高于对照组,差异均有统计学意义(P﹤0.01)。胰腺癌患者血清CA19-9水平高于食管癌、肝癌、胃癌、结直肠癌患者,差异均有统计学意义(P﹤0.05);肝癌患者血清AFP水平高于食管癌、胰腺癌、胃癌、结直肠癌患者,差异均有统计学意义(P﹤0.05)。血清CA19-9、AFP、NSE联合检测诊断消化道恶性肿瘤的AUC最大,且灵敏度、特异度及准确度均高于各指标单独检测。结论结论CA19-9、AFP、NSE在消化道恶性肿瘤患者血清中的表达水平较高,且联合检测对消化道恶性肿瘤的诊断价值较高。CA19-9、AFP对于胰腺癌和肝癌具有一定的鉴别诊断价值。Objective To explore the clinical application value of carbohydrate antigen 19-9(CA19-9),α-fetal pro-tein(AFP),neuron specific enolase(NSE)in the diagnosis of digestive tract malignancies.Method Sixty patients with digestive tract malignancies were selected as the tumor group,and 36 healthy individuals were selected as the control group.The levels of serum CA19-9,AFP,and NSE in the two groups were compared,as well as the levels of these mark-ers in patients with different digestive tract malignancies.The diagnostic value of CA19-9,AFP,and NSE single and com-bined detection for digestive tract malignancies were analyzed using the receiver operating characteristic(ROC)curve and the area under the curve(AUC).Result The levels of serum CA19-9,AFP,and NSE in the tumor group were signifi-cantly higher than those in the control group,and the differences were statistically significant(P<0.05).The level of se-rum CA19-9 in pancreatic cancer patients was higher than those in patients with esophageal cancer,liver cancer,gastric cancer,and colorectal cancer,and the differences were statistically significant(P<0.05);the level of serum AFP in liver cancer patients was higher than those in patients with esophageal cancer,pancreatic cancer,gastric cancer,and colorectal cancer,and the differences were statistically significant(P<0.05).The AUC of combined detection of serum CA19-9,AFP,and NSE in the diagnosis of digestive tract malignancies was the largest,and the sensitivity,specificity,and accura-cy were higher than those of individual marker detection.Conclusion The expression levels of CA19-9,AFP,and NSE in the serum of patients with digestive tract malignancies are relatively high,and the combined detection has a high diag-nostic value for digestive tract malignancies.CA19-9 and AFP have certain differential diagnostic value for pancreatic cancer and liver cancer.
