摘  要:目的总结人工智能(AI)技术在头颈部癌症(HNC)放射治疗各环节中的最新应用进展,探讨当前面临的挑战及未来的发展方向。方法以“人工智能、深度学习、放射治疗、头颈部癌症”作为中文关键词,以“artificialintelligence、deeplearning、radiotherapy、headandneckcancer”作为英文关键词,在PubMed、中国知网数据库中检索2014年1月-2024年7月收录的相关文献。纳入标准:(1)AI支持下的自动勾画技术;(2)AI辅助的个性化放疗计划制定;(3)AI驱动的自适应放疗流程改进;(4)AI在放疗毒性预测及预后评估中的应用。排除标准:(1)质量较低、数据不完整的文献;(2)与放射治疗相关性较低的文献。共纳入符合条件的文献82篇。结果目前,AI在HNC放射治疗自动勾画领域的应用较为成熟,可以有效减少放疗科医师的工作量并提高勾画效率。然而,在治疗计划、自适应放疗、毒性及预后预测等方面的应用,现有技术仍需进一步的验证以获得足够的临床证据。结论AI在HNC放射治疗领域显示出巨大的应用潜力,但仍面临着数据质量、算法可解释性、数据隐私与伦理等方面的挑战。随着算法的不断优化和临床研究的深入,预计AI将在提高HNC患者放射治疗疗效方面发挥愈发重要的作用。Objective To summarize the latest progress in the application of artificial intelligence(AI)technology in various aspects of radiotherapy of head and neck cancer(HNC),and to discuss the current challenges and future development directions.Methods A literature search for the period from January 2014 to July 2024 was conducted in the PubMed and CNKI databases using the keywords“artificial intelligence,deep learning,radiotherapy,and head and neck cancer”.Inclusion criteria:(1)AI-powered automatic delineation technology;(2)AI-assisted personalized radiotherapy plan formulation;(3)AI-driven adaptive radiotherapy workflow improvement;(4)Application of AI in radiotherapy toxicity prediction and prognosis assessment.Exclusion criteria:(1)Lower quality literature with incomplete data;(2)Literature with low relevance and credibility to radiation therapy.A total 82 of articles were included.Results Currently,the application of AI in the field of automatic delineation in radiotherapy of HNC is relatively mature,which can effectively reduce the workload of radiotherapy oncologists and improve the efficiency of delineation.However,the application of treatment planning,adaptive radiotherapy,toxicity and prognostic prediction,the existing technology still needs to be further validated to obtain sufficient clinical evidence.Conclusions AI has shown great potential in the field of HNC radiotherapy,but it still faces challenges in terms of data quality,algorithm interpretability,data privacy and ethics.With the continuous optimization of algorithms and the deepening of clinical research,it is expected that AI will play an increasingly important role in in improving the efficacy of radiotherapy for HNC patients.
