内窥镜辅助龈下刮治结合甘氨酸喷砂在后磨牙Ⅱ°根分叉病变治疗中的疗效评价

摘　　要：目的评价内窥镜辅助龈下刮治结合甘氨酸喷砂(GPAP)在后磨牙Ⅱ°根分叉病变治疗中的疗效。方法收集符合纳入标准且病情相似的患牙共60颗,按照同颌左右侧对照的方式配对分组:实验组在内窥镜辅助下进行龈下刮治及根面平整(SRP),并行GPAP;对照组仅使用常规SRP刮除龈下菌斑及牙石。比较术前、术后3个月两组患牙的牙周袋深度(PD)、出血指数(BI)、菌斑指数(PLI)及根分叉区水平探诊深度(HPD)的变化。结果两组患者治疗后3个月PD、BI、PLI及根分叉位点HPD均低于治疗前(P均<0.05);术后3个月研究组患者牙周指标改善优于对照组(P均<0.05);术后第1、3天实验组VAS评分低于对照组(P均<0.05);研究组治疗成功率高于对照组(P<0.05)。结论内窥镜辅助下SPR联合GPAP可改善后磨牙Ⅱ°根分叉病变的临床疗效,使更多患牙免于手术治疗,从而减少患者创伤及经济负担。Objective To evaluate the efficacy of endoscopy-assisted subgingival scaling combined with glycine powder air polishing(GAPA)in the treatment of classⅡ°furcation of posterior molars.Methods A total of 60 posterior molars with classⅡ°furcation were collected and divided into two groups according to the left and right sides of the same jaw.In the experimental group,subgingival scaling with the assistance of endoscope and GPAP were performed.In the control group,only conventional SRP was used to remove subgingival plaque and calculus.The changes of periodontal pocket depth(PD),bleeding index(BI),plaque index(PLI)and horizontal probing depth(HPD)of the root bifurcation area were compared between the two groups before surgery and 3 months after surgery.Results PD,BI,PLI and HPD of the two groups in 3 months after treatment were significantly reduced(P all<0.05).Periodontal indexes of the experimental group were better than those of the control group 3 months after surgery(P all<0.05).The VAS value of experimental group was lower than control group at the first and third day after surgery(P all<0.05).The treatment success rate of the experimental group markedly higher than that of the control group(P<0.05).Conclusion SRP combined with GPAP can improve the clinical effect of classⅡ°furcation of posterior molars,save more affected teeth from surgical treatment,and reduce the pain of trauma and economic burden of patients.
