摘 要:目的:分析结直肠癌组织中错配修复蛋白(MMR)及细胞角蛋白7(CK7)、尾型同源盒转录因子-2(CDX2)的表达与临床病理特征的相关性。方法:选取2021年1月至2023年9月,本院治疗的218例结直肠癌患者,均采集患者病灶癌组织作为观察组,以癌旁正常组织作为对照组。采用免疫组化法测定2组组织中MMR、CK7和CDX2表达水平,记录其阳性表达率,并分析其与患者临床病理特征的关系。结果:观察组CK7及CDX2阳性表达率均低于对照组(P<0.05),MMR阳性表达率高于对照组(P<0.05)。癌组织内MMR和CDX2在中高分化、非浆膜浸润型、A+B期中阳性表达率高于低分化、浆膜浸润型和C+D期(P<0.05);CK7在中高分化、非浆膜浸润型、有淋巴结转移、A+B期中阳性表达率高于低分化、浆膜浸润性、无淋巴结转移和C+D期(P<0.05);经Pearson相关性分析显示,MMR、CDX2阳性表达率与肿瘤分化程度、非浆膜浸润型呈正相关,与Dukes分期呈负相关(P<0.05);CK7与肿瘤分化、非浆膜浸润型呈正相关,与淋巴转移、Dukes分期呈负相关(P<0.05)。结论:结直肠癌患者癌组织中MMR、CK7及CDX2均呈现异常表达,其表达水平与肿瘤发生、进展存在一定相关性,可为医师准确评估患者病情提供参考。Objective To analyze the correlation between the expression of mismatch repair(MMR)proteins,cytokeratin 7(CK7),and caudal-type homeobox transcription factor-2(CDX2)and clinicopathological features in colorectal cancer tissues.Methods A total of 218 colorectal cancer patients treated between January 2021 and September 2023 were included.Cancer tissues(observation group)and adjacent normal tissues(control group)were collected.Immunohistochemistry(IHC)was used to detect the expression levels of MMR,CK7,and CDX2.Positive expression rates were recorded,and their relationships with clinicopathological features were analyzed.Results The positive expression rates of CK7 and CDX2 in the observation group were lower than those in the control group(P<0.05),and the positive expression rates of MMR were higher than those in the control group(P<0.05).The positive expression rates of MMR and CDX2 in the middle and highly differentiated,non-serous infiltrating type and A+B stage were higher than those in the low differentiated,serous infiltrating type and C+D stage(P<0.05).The positive expression rates of CK7 in middle and highly differentiated,non-serous infiltrating type,with lymph node metastasis,A+B stage were higher than those in low differentiated,serous infiltrating type,no lymph node metastasis and C+D stage(P<0.05).Pearson correlation analysis showed that the positive expression rates of MMR and CDX2 were positively correlated with the degree of tumor differentiation and non-serous infiltration type,and negatively correlated with Dukes stage(P<0.05).CK7 was positively correlated with tumor differentiation and non-serous infiltrating type,but negatively correlated with lymphatic metastasis and Dukes stage(P<0.05).Conclusion Abnormal expression of MMR,CK7,and CDX2 in colorectal cancer tissues is closely associated with tumor progression and differentiation.These biomarkers provide valuable insights for clinical evaluation and prognosis.
