MRI检查评估进展期直肠癌治疗效果及淋巴结转移的诊断价值

摘　　要：目的:探究核磁共振检查(MRI)评估进展期直肠癌治疗效果及淋巴结转移的诊断价值。方法:选取2022年1月至2023年12月,本院收治的65例进展期直肠癌患者,均行MRI检查和多层螺旋CT(MSCT)检查,以病理检查结果为金标准。分析MRI评估进展期直肠癌分期和淋巴结转移的准确性。结果:MRI检查发现N分期降低46例,T分期降低44例;MSCT检查发现N分期降低43例,T分期降低42例。两种检查的一致性较好。与病理学分期结果比较,MRI和MSCT检查对病理学T分期的判定结果一致性较好,对N分期的判定结果的一致性较差,差异无统计学意义(P>0.05)。病理检查发现淋巴结转移39例,MSCT检查发现淋巴结转移42例,MRI检查发现淋巴结转移40例。MRI检查的准确性、灵敏性和特异性均高于MSCT检查,但差异无统计学意义(P>0.05)。结论:MRI检查可对进展期直肠癌浸润程度进行判断,虽可判断淋巴结转移状况,但判断效果仍欠佳。Objective To explore the diagnostic value of magnetic resonance imaging(MRI)in evaluating the therapeutic effect of advanced rectal cancer and lymph node metastasis.Methods Total 65 patients with advanced rectal cancer admitted to our hospital from January 2022 to December 2023 were selected for MRI and Multi-slice spiral CT(MSCT)examination,with pathological findings as the gold standard to analyze the accuracy of MRI in evaluating the stage of advanced rectal cancer and lymph node metastasis.Results MRI showed that N staging decreased in 46 cases and T staging decreased in 44 cases.MSCT examination showed that N staging decreased in 43 cases and T staging decreased in 42 cases.The consistency of the two examinations was good.Compared with the results of pathological staging,MRI and MSCT showed better consistency in the determination of T staging,but poor consistency in the determination of N staging,with no statistical significance(P>0.05).Lymph node metastasis was found in 39 cases by pathological examination,42 cases by MSCT examination and 40 cases by MRI examination.The accuracy,sensitivity and specificity of MRI were higher than those of MSCT,but the difference was not statistically significant(P>0.05).Conclusion MRI can judge the infiltration degree of advanced rectal cancer.Although it can judge the status of lymph node metastasis,the judgment effect is still poor.
