摘  要:目的:探讨MRI信号强度半定量分析对肾上腺皮质腺瘤的诊断及鉴别诊断价值。方法:回顾分析经临床病理证实的78例肾上腺皮质腺瘤患者的MRI影像资料,与52例非腺瘤患者进行比较,分别测量并记录病变同相位(IP)/反相位(OP)的平均和最小信号强度,以及OP序列脾脏的平均信号强度。计算IP/OP序列病变平均衰减值(SIImean)、最小衰减值(SIImin)和OP序列病变-脾脏平均比值(ASRmean)、病变-脾脏最小比值(ASRmin),比较各值的敏感度、特异度、受试者工作特征曲线下面积(AUC),分析各值对腺瘤和非腺瘤的诊断及鉴别诊断价值。结果:SIImean诊断腺瘤的最佳阈值为22.6%,敏感度、特异度和AUC分别为91.0%、96.2%、0.955,SIImin的最佳阈值为40.0%,敏感度、特异度和AUC分别为94.9%、100%、0.989,ASRmean的最佳阈值为0.705,敏感度、特异度和AUC分别为96.2%、56.4%、0.816,ASRmin的最佳阈值为0.580,敏感度、特异度和AUC分别为98.1%、78.2%、0.930。结论:MRI信号强度半定量分析对肾上腺皮质腺瘤的诊断及鉴别诊断具有重要价值,其中,SIImin诊断腺瘤的价值更高,且操作简便、易推广。Objective:To explore the diagnostic and differential diagnostic value of semi-quantitative analysis of MRI signal intensity in adrenal cortical adenomas.Methods:The MRI data of 78 patients with adrenal cortical adenoma confirmed by clinical pathology were retrospectively analyzed and compared with 52 patients with non-adenoma.The mean and minimum signal intensity of the lesion in phase(IP)/opposed phase(OP)and the mean signal intensity of the spleen in the OP sequence were measured and recorded.The mean attenuation value(SIImean)and minimum attenuation value(SIImin)of the lesion in IP/OP sequence and the mean lesion-spleen ratio(ASRmean)and minimum lesion-spleen ratio(ASRmin)of the OP sequence were calculated.The sensitivity,specificity and area under the curve(AUC)of receiver operating characteristic(ROC)curve were compared,and the diagnostic and differential diagnostic value of each value for adenoma and non-adenoma was analyzed.Results:The best threshold of SIImean for diagnosing adenoma was 22.6%,with a sensitivity,specificity and AUC of 91.0%,96.2%and 0.955,respectively;the best threshold of SIImin was 40.0%,with a sensitivity,specificity and AUC of 94.9%,100%and 0.989,respectively;the best threshold of ASRmean was 0.705,with a sensitivity,specificity and AUC of 96.2%,56.4%and 0.816,respectively;the best threshold of ASRmin was 0.580,with a sensitivity,specificity and AUC of 98.1%,78.2%and 0.930,respectively.Conclusion:Semi-quantitative analysis of MRI signal intensity is of great value in the diagnosis and differential diagnosis of adrenal cortical adenoma,among which SIImin has a higher value in diagnosing adenoma,and is simple to operate and easy to promote.
