摘 要:目的:分析腕管综合征(CTS)的MRI及超声联合应用于该病诊断与相关参数测量及术后评价。方法:选取2021年5月—2023年12月我院收治的CTS患者57例共90侧伴有CTS的腕关节作为病例组,同期选择健康体检者45例共90侧正常腕关节作为对照组。两组均行MRI及超声检查。对比MRI及超声检测相关参数,以及不同检查方式检测CTS患者术后VAS评分以及BCTQ评分。ROC曲线分析MRI及超声联合检测在CTS诊断中的效能。结果:相比较对照组经超声、MRI检测后相关参数水平,病例组正中神经横截面积、钩骨水平正中神经前后径参数水平较高,且腕管脂肪含量多(P<0.05)。超声和MRI检查方式对CTS患者检测相关参数比较,差异无统计学意义(P>0.05)。经超声以及MRI检查后,与术前相比较,CTS患者在术后正中神经横截面积、钩骨水平正中神经前后径均减小,且腕管脂肪含量减少(P<0.05);MRI与超声检测CTS患者术后6月的相关参数比较,差异无统计学意义(P>0.05)。MRI与超声检测CTS患者术后VAS评分以及BCTQ评分比较,差异无统计学意义(P>0.05)。ROC曲线分析显示,MRI及超声联合检测对CTS诊断价值高于单项检测(P<0.05)。结论:MRI及超声联合检查可明显检测患者腕部正中神经形态以及组织结构发生的异常改变,对CTS疾病进行判断,为临床疾病诊断提供有效、便捷的方式。Objective:To analyze the combined application of MRI and ultrasound in the diagnosis,parameter measurement and postoperative evaluation of carpal tunnel syndrome(CTS).Methods:Fifty-seven patients with CTS admitted to our hospital from May 2021 to December 2023,with a total of 90 wrist joints with CTS,were selected as the case group,and 45 healthy patients with a total of 90 normal wrist joints were selected as the control group during the same period.MRI and ultrasound were performed in both groups.We compared MRI and ultrasound parameters and their efficacy in evaluating postoperative VAS and BCTQ scores in patients with CTS.ROC curve were used to analyze the efficacy combined MRI and ultrasound in the diagnosis of CTS.Results:Compared with the level of relevant parameters in the control group after detection by ultrasound and MRI,the level of parameters of median nerve cross-sectional area,anterior and posterior diameter of the median nerve at the level of the hamate bone was higher in the case group,and the carpal tunnel fat content was more(P<0.05).Comparison of ultrasound and MRI modalities for detecting relevant parameters in CTS showed no statistically significant difference(P>0.05).After ultrasound and MRI,compared with the preoperative period,the cross-sectional area of the median nerve,the anterior and posterior diameters of the median nerve at the level of the hamate bone were reduced in patients with CTS,and the fat content of the carpal tunnel was reduced(P<0.05);there was no statistically significant difference in the parameters of MRI and ultrasound in detecting patients with CTS at 6 months postoperatively(P>0.05).MRI and ultrasound detection of CTS comparison of patients’postoperative VAS scores as well as BCTQ scores showed no statistically significant difference(P>0.05).ROC curve analysis showed that the diagnostic value of combined MRI and ultrasound detection for CTS was higher than that of single detection(P<0.05).Conclusion:The combined MRI and ultrasound examination can effectively detec
