CO_(2)激光术联合复方斑蝥胶囊治疗早期声门型喉癌临床价值研究

摘　　要：目的:探究CO_(2)激光术联合复方斑蝥胶囊治疗早期声门型喉癌的临床价值。方法:选取80例早期声门型喉癌患者,随机分为手术组(40例)和联合组(40例)。手术组给予CO_(2)激光术联合含0.9%氯化钠溶液胶囊治疗,联合组给予CO_(2)激光术联合复方斑蝥胶囊治疗。比较两组治疗前及治疗后1年嗓音功能、免疫功能、吞咽功能及生活质量。两组治疗后随访1年,记录患者复发情况。结果:治疗后1年,两组患者谐噪比高于治疗前,基频微扰、振幅微扰低于治疗前,且联合组谐噪比高于手术组,基频微扰、振幅微扰低于手术组(均P<0.05)。治疗后1年,手术组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平低于治疗前,联合组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平高于治疗前,且联合组高于手术组(均P<0.05)。治疗后1年,两组吞咽功能评估量表(GUSS)评分和功能、总体生命质量评分高于治疗前,症状评分低于治疗前,且联合组GUSS评分和功能、总体生命质量评分高于手术组,症状评分低于手术组(均P<0.05)。两组治疗后1年复发率比较差异无统计学意义(P>0.05)。结论:CO_(2)激光术联合复方斑蝥胶囊治疗早期声门型喉癌能够促进嗓音功能恢复,改善免疫及吞咽功能,提高患者生活质量。Objective:To explore the clinical value of CO_(2)laser surgery combined with compound cantharidin capsules in the treatment of early glottic laryngeal cancer.Methods:A total of 80 patients with early glottic laryngeal cancer were selected and randomly divided into an operation group(40 cases)and a combination group(40 cases).The operation group received CO_(2)laser surgery combined with capsules containing 0.9%sodium chloride solution,while the combination group received CO_(2)laser surgery combined with compound cantharidin capsules.Voice function,immune function,swallowing function and quality of life were compared between the two groups before and one year after treatment.Both groups were followed up for one year post-treatment,and recurrence rates were recorded.Results:One year after treatment,the harmonic-to-noise ratio was higher and the jitter and shimmer were lower in both groups compared to before treatment,and the combination group had a higher harmonic-to-noise ratio and lower jitter and shimmer than the operation group(all P<0.05).One year after treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were lower in the operation group and higher in the combination group compared to before treatment,with the combination group showing higher levels than the operation group(all P<0.05).One year after treatment,the Gugging Swallowing Screen(GUSS)scores,functional and overall quality of life scores in both groups were higher than before treatment,while symptom scores were lower than before treatment.The combination group had higher GUSS scores,functional and overall quality of life scores,and lower symptom scores compared to the operation group(all P<0.05).There was no significant difference in the recurrence rate between the two groups one year after treatment(P>0.05).Conclusion:CO_(2)laser surgery combined with compound cantharidin capsules in the treatment of early glottic laryngeal cancer can promote the recovery of voice function,improve immune and swallowing functions,and enhance the quality of life
