摘　　要：目的:探讨葡萄糖转运蛋白1(GLUT1)、人附睾蛋白4(HE4)和胰岛素样生长因子-1(IGF-1)在卵巢癌组织中的表达及临床价值。方法:收集110例接受手术治疗的卵巢癌患者术中癌组织和癌旁组织标本,免疫组织化学法检测卵巢癌及癌旁组织GLUT1、HE4、IGF-1蛋白表达。分析卵巢癌组织中GLUT1、HE4、IGF-1蛋白表达与患者临床病理特征的关系。随访3年,根据患者癌因死亡情况分为预后良好组(72例)和预后不良组(38例)。比较不同预后患者卵巢癌组织GLUT1、HE4、IGF-1蛋白表达,行Cox回归分析预后的影响因素。结果:卵巢癌组织中GLUT1、HE4、IGF-1蛋白阳性表达率高于癌旁组织(均P<0.05)。GLUT1、HE4、IGF-1蛋白在FIGO分期、肿瘤分化程度及淋巴结转移方面阳性表达率比较差异有统计学意义(均P<0.05)。预后不良组GLUT1、HE4、IGF-1蛋白阳性表达率高于预后良好组(均P<0.05)。淋巴结转移、肿瘤低分化、GLUT1、HE4及IGF-1蛋白阳性表达为卵巢癌患者预后的独立危险因素(均P<0.05)。结论:卵巢癌组织中GLUT1、HE4、IGF-1蛋白高表达,且与患者FIGO分期、淋巴结转移、肿瘤分化程度和预后有关。Objective:To investigate the expression and clinical value of glucose transporter 1(GLUT1),human epididymis protein 4(HE4)and insulin-like growth factor-1(IGF-1)in ovarian cancer tissue.Methods:Tumor and adjacent non-tumor tissue samples were collected from 110 patients with ovarian cancer who underwent surgical treatment.The expression of GLUT1,HE4 and IGF-1 proteins in ovarian cancer and adjacent tissues was detected by immunohistochemistry.The relationship between the expression of GLUT1,HE4 and IGF-1 proteins in ovarian cancer tissue and the clinical pathological features of patients was analyzed.Patients were followed up for 3 years and divided into good prognosis group(72 cases)and poor prognosis group(38 cases)based on cancer-related mortality.The expression of GLUT1,HE4 and IGF-1 proteins in ovarian cancer tissue was compared between patients with different prognoses,and Cox regression analysis was performed to identify the factors affecting prognosis.Results:The positive expression rates of GLUT1,HE4 and IGF-1 proteins in ovarian cancer tissue were higher than those in adjacent non-tumor tissue(all P<0.05).The positive expression rates of GLUT1,HE4 and IGF-1 proteins were significantly different in terms of FIGO stage,tumor differentiation and lymph node metastasis(all P<0.05).The positive expression rates of GLUT1,HE4 and IGF-1 proteins in the poor prognosis group were higher than those in the good prognosis group(all P<0.05).Lymph node metastasis,low tumor differentiation,and positive expression of GLUT1,HE4 and IGF-1 proteins were identified as independent risk factors for prognosis in patients with ovarian cancer(all P<0.05).Conclusion:High expression of GLUT-1,HE4 and IGF-1 proteins in ovarian cancer tissues is associated with FIGO stage,lymph node metastasis,tumor differentiation and prognosis of patients.
