摘  要:目的探究视神经脊髓炎(NMO)患者灰、白质静息态脑功能的改变与认知障碍的关联机制。方法采集46例NMO患者及52名健康对照者(NC组)功能磁共振成像(fMRI)图像及进行神经心理学评估。基于MATLAB的DPARSFA工具包和白质定制分析管道进行数据分析处理,采用双样本t检验进行组间比较,提取差异脑区和白质束的比率低频振幅(fALFF)、局部一致性(ReHo)值与NMO患者的认知分数进行相关性分析。结果与NC组相比,NMO患者不仅在海马旁回、颞中回、枕中回等灰质区域ReHo值均增强,在扣带束、额枕上束、后放射冠等白质束fALFF和ReHo值也出现增强/降低。其中右侧枕中回、角回等区域ReHo值升高与Stroop评分(P=0.004)及TMT-A评分(P=0.014)呈显著负相关;左侧额枕上束与Stroop评分显著负相关(P=0.033)。结论NMO患者认知损害与灰、白质自发神经活动的重塑密切相关。Objective To explore the association mechanism between changes in resting-state brain function in gray and white matter and cognitive impairment in patients with optic neuromyelitis(NMO).Methods fMRI images were collected from 46 NMO patients and 52 normal controls,and neuropsychological evaluations were performed.Data were analyzed using the MATLAB DPARSFA toolkit and a customized white matter analysis pipeline.Two-sample t-tests were used for between-group comparisons,and the fALFF and ReHo values of differential brain regions and white matter tracts were extracted and correlated with cognitive scores of NMO patients.Results Compared with the normal control group,NMO patients showed enhanced ReHo values in gray matter regions such as the parahippocampal gyrus,middle temporal gyrus,and middle occipital gyrus,as well as altered fALFF and ReHo values in white matter tracts such as the cingulum,superior fronto-occipital fasciculus,and posterior corona radiata.Specifically,increased ReHo values in the right middle occipital gyrus and angular gyrus were significantly negatively correlated with Stroop test scores(P=0.004)and TMT-A scores(P=0.014).The left superior fronto-occipital fasciculus also showed a significant negative correlation with Stroop test scores(P=0.033).Conclusion Cognitive impairment in NMO patients is closely related to the remodeling of spontaneous neural activity in both gray and white matter.
