摘 要:目的探讨体素内不相干运动扩散加权成像(IVIM-DWI)定量参数与乳腺癌组织中缺氧诱导因子1α(HIF-1α)表达的相关性并评估其预测HIF-1α表达水平的效能。方法回顾性分析经病理证实且行IVIM-DWI扫描的乳腺癌患者104例,其中HIF-1α高表达型63例,低表达型41例。利用GE后处理工作站计算表观扩散系数(ADC)、单纯扩散系数(D)、灌注相关扩散系数(D^(*))及灌注分数(f)。比较两组间各定量参数值的差异,绘制受试者操作特征(ROC)曲线评价各定量参数值的预测效能。结果D值与HIF-1α表达水平呈负相关,D^(*)和f值与HIF-1α表达水平呈正相关,差异均有统计学意义(P<0.05)。HIF-1α低表达组中D值高于高表达组,而其D^(*)和f值均低于高表达组,差异有统计学意义(P<0.05)。D、D^(*)和f值的ROC曲线下面积(AUC)差异不具有统计学意义(Z=0.687~1.414,P>0.05),但IVIM-DWI定量参数联合模型诊断效能均高于单一参数值,其AUC为0.892(Z=3.203,3.685,3.998;P<0.05)、敏感度为75.61%、特异度为88.89%和准确率为79.8%。结论D值与HIF-1α表达呈负相关,D^(*)和f值与HIF-1α表达呈正相关。D、D^(*)和f值可用于HIF-1α表达水平的预测,有助于为乳腺癌患者治疗选择和预后评估等提供新的影像学依据。Objective To investigate the correlation between intravoxel incoherent motion diffusion-weighted imaging(IVIM-DWI)quantitative parameters and the expression of hypoxia-inducible factor 1α(HIF-1α)in breast cancer,and to evaluate their predictive efficiency.Methods A retrospective study was conducted on 104 breast cancer patients whose diagnoses were confirmed by pathology and who underwent IVIM-DWI scans.Among these patients,63 had high HIF-1αexpression,while 41 had low expression.The apparent diffusion coefficient(ADC),pure diffusion coefficient(D),perfusion-related diffusion coefficient(D^(*)),and perfusion fraction(f)were calculated using a GE post-processing workstation.Differences in quantitative parameter values between the two groups were compared,and receiver operating characteristic(ROC)curves were generated to assess the predictive efficiency of each parameter.Results The D value was negatively correlated with HIF-1αexpression levels,while D^(*)and f values were positively correlated with HIF-1αexpression levels,with statistically significant differences(P<0.05).In the low HIF-1αexpression group,the D value was higher than in the high expression group,while D^(*)and f values were lower,with statistically significant differences(P<0.05).Differences in AUC values for D,D^(*),and f were not statistically significant(Z=0.687 to 1.414,P>0.05).However,the combined IVIM-DWI quantitative parameter model had higher diagnostic efficiency than single parameter values,with an AUC of 0.892(Z=3.203,3.685,3.998;P<0.05),sensitivity of 75.61%,specificity of 88.89%,and accuracy of 79.8%.Conclusion The D value is negatively correlated with HIF-1αexpression,while D^(*)and f values are positively correlated with HIF-1αexpression.D,D^(*),and f values can be used to predict HIF-1αexpression levels,providing new imaging evidence for treatment selection and prognosis assessment in breast cancer patients.
