摘　　要：炎症性肠病(IBD)是一组病因尚未完全明确的非特异性慢性胃肠道炎症性疾病。IBD合并心理疾患的问题近年来受到广泛关注,其可影响患者生活质量,同时增加IBD复发风险,导致不良预后。肠⁃脑轴参与了IBD合并神经精神心理疾患的发生,肠道可通过神经系统、下丘脑⁃垂体⁃肾上腺(HPA)轴和免疫系统调节大脑功能。肠道微生态则参与了肠道炎症和中枢神经系统免疫环境的形成。本文就肠道免疫和微生态与IBD患者合并心理疾患的联系,以及基于肠道免疫和微生态的心理疾患治疗策略作一综述,以期为IBD合并心理疾患的诊疗提供新思路。Inflammatory bowel disease(IBD)is a group of non⁃specific chronic gastrointestinal inflammatory disorders with incompletely elucidated etiology.The comorbidity of IBD and psychological disorders has garnered increasing attention in recent years,as it affects the patients′quality of life,increases the risk of disease relapse and contributes to poor prognosis.The gut⁃brain axis plays a pivotal role in development of IBD⁃associated neuropsychiatric comorbidities.The gut modulates brain function through the nervous system,the hypothalamic⁃pituitary⁃adrenal(HPA)axis,and the immune system.Additionally,the intestinal microecology participates in shaping both intestinal inflammation and the immune environment of the central nervous system.This article reviewed the relationship between intestinal immunity,intestinal microecology and the psychological disorders in IBD patients,as well as the therapeutic strategies targeting intestinal immunity and microecology for psychological disorders.These findings may provide novel insights into the diagnosis and treatment of psychological comorbidities in IBD.
