摘  要:目的探究肺癌应用神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、鳞癌相关抗原(SCCAg)、糖类抗原125(CA125)、细胞角蛋白19的可溶性片段(CYFRA21-1)检测的诊断价值。方法纳入78例均为2020年7月至2023年8月于周口市人民医院就诊的肺癌患者入肺癌组,选取同期肺部良性病变患者40例入良性组,另选取健康体检者30名入对照组。均抽取静脉血测定血清NSE、CEA、SCCAg、CA125、CYFRA21-1水平。对比2组各血清肿瘤标志物水平、不同类型肺癌患者血清肿瘤标志水平,另分析各指标单项与联合检测在肺癌中的诊断价值。结果相比于对照组,肺癌组与良性组患者各血清肿瘤标志物水平均较高,且肺癌组高于良性组(P<0.05)。相比于对照组,肺腺癌、肺鳞癌及小细胞肺癌患者各血清肿瘤标志物水平较高,且小细胞肺癌患者NSE、CA125水平均高于肺鳞癌、肺腺癌,CEA、SCCAg、CYFRA21-1水平均低于肺鳞癌、肺腺癌(P<0.05)相比于对照组,肺腺癌、肺鳞癌及小细胞肺癌患者各血清肿瘤标志物水平较高(P<0.05)。受试者工作特征(ROC)曲线显示:联合检测在肺癌中的诊断价值最高,曲线下面积(AUC)为0.924,灵敏度为0.949,特异度为0.750;其次为NSE、SCCAg、CYFRA21-1、CEA、CA125。结论肺癌诊断中应用各血清肿瘤标志物联合检测具有较高的临床应用价值,在临床诊断中可结合多种血清肿瘤标志物联合检测的方式以提升肺癌诊断效能,对评估患者预后意义重大。Objective To explore the diagnostic value of detecting neuron-specific enolase(NSE),carcin-oembryonic antigen(CEA),squamous cell carcinoma-associated antigen(SCCAg),carbohydrate antigen 125(CA125),and soluble fragments of cytokeratin 19(CYFRA21-1)in lung cancer.Methods A total of 78 lung cancer patients who received treatment at Zhoukou People′s Hospital from July 2020 to August 2023 were included in the lung cancer group.Forty patients with benign lung lesions during the same period were selected as the benign group,and 30 healthy individuals who underwent physical examinations were selected as the control group.Venous blood samples were collected to measure serum levels of NSE,CEA,SCCAg,CA125,and CYFRA21-1.Compare the levels of various serum tumor markers between two groups and different types of lung cancer patients,and analyze the diagnostic value of single and combined detection of each indicator in lung cancer.Results Compared with the control group,the levels of various serum tumor markers were higher in both the lung cancer group and the benign group,and the lung cancer group was higher than the benign group(P<0.05).Compared with the control group,patients with lung adenocarcinoma,lung squamous cell carcinoma,and small cell lung cancer had higher levels of serum tumor markers.The levels of NSE and CA125 in small cell lung cancer patients were higher than those in lung squamous cell carcinoma and lung adenocarcinoma,while the levels of CEA,SCCAg,and CYFRA21-1 were lower than those in lung squamous cell carcinoma and lung adenocarcinoma(P<0.05).The receiver operator characteristic curve(ROC)showed that combined detection had the highest diagnostic value in lung cancer,with the area under the curve(AUC)of 0.924,sensitivity of 0.949 and specificity of 0.750.It was followed by NSE,SCCAg,CYFRA21-1,CEA and CA125.Conclusion In the diagnosis of lung cancer,the combined detection of various serum tumor markers has high clinical application value,and the combined detection of multiple serum tumor markers can be co
