摘 要:目的比较宫颈癌患者盆腔骨髓保留调强放疗(PBMS-IMRT)与常规IMRT放疗靶区的剂量体积和盆腔骨髓的毒性。方法选取60例宫颈癌术后辅助放化疗适应证的患者,分为IMRT组(n=30)和PBMS-IMRT组(n=30),观察并记录两组的计划参数、危及器官剂量体积值以及血液细胞值,评估血液毒性及其严重程度。结果两组间目标区一致性指数(CI)、同质性指数(HI)差异无统计学意义(P>0.05)。PBMS-IMRT组的骨盆骨V_(20)-V_(50)低于IMRT组(P<0.05),PBMS-IMRT组的血液学毒性较低(P<0.05)。结论PBMS-IMRT显著降低骨盆骨髓的剂量,从而降低患者血液毒性。Objective To compare the dose volume of the target area and the toxicity of pelvic bone marrow-sparing intensity-modulated radiotherapy(PBMS-IMRT)with routine IMRT in patients undergoing radiochemotherapy for cervical cancer.Methods 60 patients with postoperative indications of adjuvant chemoradiotherapy for cervical cancer were selected and divided into IMRT group(n=30)and PBMS-IMRT group(n=30).The planned parameters,dose volume value of organs at risk and blood cell value of the two groups were observed and recorded to evaluate blood toxicity and its severity.Results There was no significant difference in the consistency index(CI)and homogeneity index(HI)between the two groups(P>0.05).The pelvic bone V_(20)-V_(50) of the PBMS-IMRT group was lower than that of the IMRT group(P<0.05),and the hematological toxicity of the PBMS-IMRT group was lower(P<0.05).Conclusion PBMS-IMRT significantly reduces the dose of pelvic bone marrow,thereby reducing blood toxicity in patients.
