摘 要:目的 探讨并分析乳酸杆菌在宫颈癌患者发生、发展中的作用及其与Th1和Th2类细胞因子的关系。方法 选择2021年1月至2024年1月在保定市第二中心医院就诊的80例宫颈癌患者作为观察组,根据患者是否存在人乳头瘤病毒(HPV)感染将其分为HR-HPV组(32例)和非HR-HPV组(48例),并选择同时期在我院体检为健康的80例女性为对照组,对所有参与研究的女性进行阴道内乳酸杆菌及血清Th1和Th2类细胞因子水平检测,比较并分析3组间各指标差异。结果HR-HPV组的IFN-γ(19.88±1.64) vs (26.12±2.23)、IL-2(20.76±1.77) vs(24.51±1.86)、CD4^(+)细胞数量(19.47±6.42) vs (24.12±7.04)及CD4^(+)/CD8^(+)比值(0.61±0.12) vs (0.75±0.10)显著低于非HR-HPV组;HR-HPV组的IL-4(49.02±4.87) vs (37.54±3.41)和IL-6(54.21±5.31) vs (46.46±4.27)水平显著高于非HR-HPV组。Logistic回归分析结果表明,乳酸杆菌阳性(OR=0.571, 95%CI:0.396~0.822)、IFN-γ(OR=0.964, 95%CI:0.949~0.979)和CD4^(+)/CD8^(+)比值(OR=0.629, 95%CI:0.432~0.916)为宫颈癌的保护因素,而IL-4(OR=2.326, 95%CI:1.713~3.157)则为危险因素。结论 宫颈癌患者存在较高的HPV感染率,存在阴道内乳酸杆菌菌群失调及机体内Th1/Th2类细胞因子失调,HPV感染会加重患者阴道内乳酸杆菌菌群失调程度及机体内Th1/Th2类细胞因子失调程度,且HPV感染、阴道内乳酸杆菌菌群失调及机体内Th1/Th2类细胞因子失调对引发宫颈癌的发生促进作用,并与宫颈癌分期和淋巴转移相关。Objective To explore and analyze the role of lactobacilli in the occurrence and development of cervical cancer and their relationship with Th1 and Th2 cytokines in patients.Method From January 2021 to January 2024,80 cervical cancer patients who visited our hospital were selected as the observation group.They were divided into an HR-HPV group(n=32)and a non HRHPV group(n=48)based on the presence of HPV infection.At the same time,80 healthy women who underwent physical examinations in our hospital during the same period were selected as the control group.The levels of vaginal lactobacilli and serum Th1 and Th2 cytokines were detected in all participating women in the study,and the differences in various indicators among the three groups were compared and analyzed.Results In the HR-HPV group,the levels of IFN-γ(19.88±1.64)vs(26.12±2.23),IL-2(20.76±1.77)vs(24.51±1.86),CD4^(+)cell count(19.47±6.42)vs(24.12±7.04),and CD4^(+)/CD8^(+)ratio(0.61±0.12)vs(0.75±0.10)were significantly lower compared to the non-HR-HPV group.Conversely,the levels of IL-4(49.02±4.87)vs(37.54±3.41)and IL-6(54.21±5.31)vs(46.46±4.27)were significantly higher in the HR-HPV group than in the non-HR-HPV group.Logistic regression analysis showed that lactobacillus positivity (OR =0.571, 95% CI: 0.396-0.822), IFN-γ (OR =0.964, 95% CI:0.949-0.979) and CD4^(+)/CD8^(+) ratio (OR =0.629,95% CI:0.432-0.916) were protective factors in cervical cancer, while IL-4 (OR=2.326, 95% CI:1.713-3.157) was a riskfactor. Conclusions Patients with cervical cancer exhibit a high rate of HPV infection, an imbalance of vaginal lactobacillusflora, and a dysregulation of Th1/Th2 cytokines. HPV infection exacerbates the imbalance of vaginal lactobacillus flora and thedysregulation of Th1/Th2 cytokines in the body. HPV infection, the imbalance of vaginal lactobacillus flora, and the dysregulationof Th1/Th2 cytokines collectively promote the development of cervical cancer and are associated with the staging andlymphatic metastasis of the disease.
