摘  要:目的探究幽门螺杆菌(Helicobacter pylori,H.pylori)感染对不同程度慢性胃炎患者胃内真菌及细菌丰度及多样性的影响。方法收集我院H.pylori阳性患者的胃黏膜标本作为试验组,分为:H.pylori阳性慢性非萎缩性胃炎(non-atrophic gastritis,CNG)组(n=20)、H.pylori阳性慢性萎缩性胃炎(chronic atrophic gastritis,CAG)+组(n=20)、H.pylori阳性CAG++/+++伴/不伴肠上皮化生(intestinal metaplasia,IM)组(n=9),另选取H.pylori阴性CNG患者的胃黏膜标本作为对照(n=6)。对真菌ITS 3~4区域及细菌16S rDNA V3~V4区域进行扩增后行高通量测序,分析真菌菌株进化分支及细菌多样性、丰度与H.pylori感染慢性胃炎(chronic gastritis,CG)患者胃黏膜损伤的关系。结果胃内真菌菌群相比较,四组间Chao1指数差异有统计学意义(F=3.166,P=0.032),而Shannon指数在H.pylori阳性CG三组间逐渐升高,但差异无统计学意义(F=1.826,P=0.172)。CG四组间胃内细菌菌群相比较,Shannon指数差异有统计学意义(F=3.240,P=0.030),而Chao1指数在H.pylori阳性CG三组间逐渐降低,但差异无统计学意义(F=0.554,P=0.648)。应用PCoA主坐标分析,使用加权Unifrac距离分析,四组患者胃内真菌出现明显聚类,但仍可见不完全重合部分;而细菌菌群出现聚类,但可明显分开,表明CG四组间真菌及细菌组成具有差异。根据菌群分析,在属水平上,真菌Malassezia及Trichoderma在CAG组中富集;细菌Ralstonia在CAG组中比例显著高于CNG组,Helicobacter在CNG组中的比例高于CAG组。结论在H.pylori阳性CG发生发展过程中,H.pylori丰度降低,且H.pylori感染会显著降低胃内细菌菌群分布多样性,真菌菌群分布丰度增加,且真菌及细菌菌群组成和结构发生明显改变。Objective To explore the effects of Helicobacter pylori(H.pylori)infection on the abundance and diversity of gastric fungi and bacteria in patients with different degrees of chronic gastritis.Methods Collect gastric mucosal specimens from H.pylori positive patients in our hospital as the experimental group,divided into:H.pylori positive chronic non-atrophic gastritis(CNG)group(n=20),H.pylori positive chronic atrophic gastritis(CAG)+group(n=20),H.pylori positive CAG++/+++with/without intestinal metaplasia(IM)group(n=9),and select gastric mucosal specimens from H.pylori negative CNG patients as controls(n=6).After amplification of fungal ITS 3-4 regions and bacterial 16S rDNA V 3-V 4 regions,high-throughput sequencing was performed to analyze the evolutionary branches of fungal strains and the relationship between bacterial diversity and abundance and gastric mucosal damage in patients with chronic gastritis(CG)infected with H.pylori.Results Compared with the fungal flora in stomach,Chao1 index showed a statistically significant difference in four groups(F=3.166,P=0.032),while Shannon index gradually increased among the three groups with H.pylori positive CG,but the difference was not statistically significant(F=1.826,P=0.172).The difference of Shannon index in gastric bacterial flora among the four CG groups was statistically significant(F=3.240,P=0.030),while the Chao1 index gradually decreased among the three groups with H.pylori positive CG,but the difference was not statistically significant(F=0.554,P=0.648).By PCoA principal coordinate analysis and weighted Unifrac distance analysis,gastric fungi in the four groups showed obvious clustering,but incomplete overlap was still visible.The bacterial flora showed clustering,but could be clearly separated,indicating that the composition of fungi and bacteria was different among the four CG groups.According to the flora analysis,the fungi Malassezia and Trichoderma were enriched in CAG group at genus level.The proportion of Ralstonia in CAG group was significantly hi
